7 Years of Pap Therapy and Still Utterly Fatigued. Top doctors Unhelpful
Symptoms:
Extreme daytime fatigue. Morning and afternoon headaches. Nighttime urination. Not feeling well rested or rejuvenated.
Pap Therapy helps, but a negligible amount.


Brief Timeline:
  • Kaiser at home sleep study in December 2016
  • Placed on CPAP with Resmed AirSense 10
  • Tried all different masks, liners, prodding settings, etc.
  • Stanford titration (split night) study in June 2021
  • Placed on Bipap with AirCurve 10 VAuto with recommended settings: IPAP 10, EPAP 5, PS 3 
  • Again tried all different masks, settings, etc.
  • Went to more top Stanford doctors
  • Stanford Sleep study (no pap therapy ~3 days before) in April 2022
  • Went to top ENT sleep surgeons at Stanford and gaslight - everything seems fine. (I can pull up the exact notes if desired)
  • Deviated septum fixed - septoplasty and inferior turbinate reduction in February 2024

Current Settings:
Resmed AirCurve 10 vAuto
5.8 over 8.6 with PS 2.8
AirFit N30i nasal pillows mask - my lower nostrils have sores from it
Taping mouth with medical cover roll stretch tape
Sleep Mask for morning light

Try increasing PS to 3.6 and trigger sensitivity to high...just a hunch.

I wonder if there are any other underlying health issues causing sleep apnea 

 2nd I wonder if you try modafinil a prescription for excessive daytime sleepiness. 

3 - Sleep Study - In Lab - did it say anything about narcolepsy or other sleep disorders
