Check it out! You matched my settings. Try again, and to get rid of the CA events change the Trigger Sensitivity setting to very-high. The clustered OA is positional from chin-tucking, but not a concern at this frequency. The faster trigger may smooth out the inspiratory peaks, and if not, we can add a few tenth of PS. Let's keep changes to a minimum and work out the details. It's up to you whether to use the nasal or full-face, but take a look at the F40 and you might see it is a hybrid that could work for you.

This was the next night with the settings PS 4.0 over 8-20 with trigger set at high. Still getting those flat caps on inspiratory, although my CA events did go down.

Increase PS to 4.4, trigger very-high and Ti Max 2.6.

I appreciate your help and attention to this. I've titrated up before so I have some historical data to show you. Here's my OSCAR data at S-Mode 10/14 and I'm still getting the caps and peaks. Would that affect your prior recommendation as this was at an even higher pressure?

Does nothing to affect my opinion. You didn't need the higher pressure, and it is the pressure support that smooths out the inspiratory peaks. Pressure alone is not a solution to flow limitation, or we would use CPAP. It would be better to use Vauto mode to get a reliable statistic on flow limitation. There is no problem with limiting the maximum pressure, as long as it accommodates the PS you input. Check your Oscar version and update to 1.6.0 released today.

Okay so try PS 4.4 over 8-20, trigger set at very high, and Ti max 2.6 - do I have this correct? Will update OSCAR as well.

Hi Mx117 ,

I've been at that low place too and it had a lot to do with finding the right mask. I think i have finally come to a tape solution that works. 

What kind of tape are you using and how large? 

Some tapes do not sustain adequate adherence to the skin and some are so strong that they cause abrasions when removed too quickly. Others are so thin that they leak even though they are in place.

My solution is to place a full strip of lighter tape completely over my mouth and place a small piece of more occlusive tape over the middle of my mouth where air tends to leak out. This way, the lighter, thinner tape can be removed easily and the stronger tape comes off with it (rather than pulling on the skin too much. Have you tried Cover-roll stretch tape? It may work for you all by itself. 

The right mask for YOU is worth finding. I've tried six and have settled on the F&P Solo. It's like searching for the Holy Grail, for sure.
