My SD card is full. Do I need to back it up before reformatting or is the OSCAR backup adequate?

I would recommend making a backup of your SD card, because the OSCAR backup only contains the files that OSCAR uses, not the complete content of the card.
Apnea Board Monitors are members who help oversee the smooth functioning of the Board. They are also members of the Advisory Committee which helps shape Apnea Board's rules & policies. Membership in the Advisory Members group does not imply medical expertise or qualification for advising Sleep Apnea patients concerning their treatment.

I would make a backup of the SD card onto the computer (or cloud, external drive, etc.).

You can also get a second SD card to use.  Anything from 2GB to 32Gb will work. Then keep the original SD card in a safe place.
