Thank you for all your help. Ill try increasing the min pressure to 11.
I'm unsure what happened for that 1 hour period. I remember feeling a bit frustrated breathing with the CPAP. It may be me adjusting to having removed the ramp.
Since starting CPAP, i have been waking up more than often and waking up earlier. The waking through the night appears to coincide with REM.

RE: Prisma 20A - Change of machine and regression
Hi All,
Apologies for the lack of communication... Busy over the Christmas period and settling back into work.
I've made the following changes:
I bought a 15mm hose and have been using a fixed pressure of 13.5 with a softpap of 3
I bought an Airtouch N20 nasal mask and have been using mouthtape - this was the biggest gamechanger for lowering AHI
Bought a buckwheat pillow - another big gamechanger
I still feel like i can make more improvements, however i feel like im on the right track. See screenshots of the past few nights below. The first 2 werent the best due to heat and having to travel briefly on the following day.
If someone could please give me some more advice that would be much appreciated.
I would suggest you to disable softpap, if you have not tried it? For some people reras and flow limitations get worse with softpap on, including myself.
I have a lot of experience on the Smart Max and 20A/S model.
My doctor also put me on softpap1, but I sleep much better without softpap 1-3.
Sometimes higher pressure is not the solution.
I would go back and try APAP around 7-13
NO softpap
NO ramp
Standard setting alghorithm
Remember to change hose settings from 21 mm to 15mm in settings.
I do not see why you would run 15mm, if the size is not an issue. The machines comes standard with 21mm hoses.
Take a look at this wiki about optimizing the Lowenstein. This article is a work in progress, and I'm trying to get a better handle on the Lowenstein settings. https://www.apneaboard.com/wiki/index.ph...ptimiztion

RE: Prisma 20A - Change of machine and regression
In my signature is the link for the Prisma info, the page location was changed. Be good to clean up your Oscar display a little so we can see more useful info.
Softpap 3 will give the best bi-level therapy you will get from the Prisma 20a. I found the larger hose to be much more comfortable on that machine due to the valve controlled air delivery which means the pump is always spooled and can release air at you very abruptly. I believe certain UARS sufferers don't tolerate the strong instant flow and can benefit from this being momentarily slowed at the beginning of inspiration by a vcom. Makes night and day difference to me.
