RE: Sleepy, too many arousals?
in the long run, your sensitive area that the pillows reach will need to toughen up. there are a few things you can try.

1. aloe gel
2. something like aquaphor. i am skeptical, seeing as it is petroleum based, but by all accounts, it is safe to use. whether it's bad for the silicone, i don't know.
3. and this is the weirdest...you can use nipple cream. it's basically lanolin.

i gave up after a few tries and went to a nasal cradle. but if you can take the pain for a week or so, it's supposed to get better.

good luck.

eok361, thanks for the tips, yeah that weirdest one is the nipple cream, but being Lanolin is good. I have the nasal cradle, but want to try the pillow if I can handle it...

RE: Sleepy, too many arousals?
Maybe my arousals impact my REM sleep, I can't figure this out myself.  Even last night's sleep has little Rem sleep in the first part of the night. Thanks again for reviewing and providing advice.

Pressure change going from a Nasal cradle to pillow mask
I need some advice please. I am changing to the ResMed P10 nasal pillow cushion from the ResMed N30 nasal cradle cushion.
I have received some god tips about using lanoline to help my nose not be irritated so much.
But I forgot to ask if I should lower my pressure a little bit since the air might be better directed into my nose. I use constant pressure of 10.2 on a ResMed AirSense 11 in the cpap mode. Should I try 10 or 9.8, not making a huge change obviously?
I use a 5" long piece of Kinesiology tape, 4 inches wide, and a chin strap also. I just got the Caldera soft neck cushion, but it is a bit small. Even with using the last little bit of velcro, there is too much pressure on my neck. I woke up mumbling with a sore adam's apple. Hopefully some velcro extension will help.
Thanks for any help in the pressure settings. My breathing was better even though the neck collar was too tight. I took it off after waking up and had a bit more leaks.

When there is a leak, the instrument maintains the pressure by increasing the flow. You should not alter the pressure setting unless you had some enormous leaks before that compromised your treatment. Looking at your chart, there were some instances when the leak was over the limit, but the average was acceptable. I do not suggest altering the pressure because of the mask changes.

I am concerned about the extremely tight collar you use, which is restricting blood flow in your neck. You should not use this collar further.

Thanks for the concern about the tight collar, but I have an velcro extension coming, so hopefully I can use it again then. As you said, never a good idea to restrict blood flow in you neck. Thanks!
I was wondering if the pillow type cushions direct the air so much more accurately, that a bit lower air pressure would provide an equivalent apnea relief as other types of cushions at a higher pressure. In my particular case I seem to have a better night of rest when the pressure is lower. Originally in 2018, I was on a fixed pressure of 15 with a full face mask. I did not have Oscar until this summer, but I had loud leaks back then. With my newer nasal cradle cushion, I am able to lower the fixed  pressure to around 10.
Last night was worse without the collar. Neighbors complained about my snoring, haha.

Your flow limit is out of the acceptable range. I suggest running a screening test in the APAP regime to see the pressure you are targeting. If you are willing to do that, I can provide further details on the setting.

G. Szabo,
Thanks for the fast and detailed response.
Please see my attached sleep reports.

Last night's sleep went to crap with my neck being a bit sore. I am having major neck surgery in Jan 2025. I think the servical osft

At this time i am apprehensive about the APAP regime. I have been on the CPAP mode since 2018, on Dream station 1 and 2.

If you do not want to run APAP screening, you should calibrate the pressure and EPR step by step over a few weeks to determine the optimum range.
Your high flow limit indicates that your pressure needs to be adjusted, and some EPR should be set.
I would suggest starting by increasing your EPR to 1.

G. Szabo,
I am switching to the P10 pillow cushion tonight.
I will use a soft cervical collar as soon as the vecro extension comes in.
I will check the flow limits and see how it goes.
Do you still think I should turn on EPR?
Thanks for you time and advice.
