SR: Just came in from shopping all day and no messages left as of 3:17PM.  At 9:00AM I had called the Dr's Office and left a message requesting the 13/25 PS-4.  If I don't get a response tonight (doubtful) then give me your shopping list if it is more over the top than that above.  I'm thinking of taking an overnight 'red-eye' airplane ride tonight.


Can you explain why the Sleep Report screen was showing 10-13 when I thought I was suffocating last night as max IPAP was set at 20?  Given this what will the IPAP of 25 drop to?

On CPAP, your pressure was pretty consistently pegged at your maximum set pressure of 19 (EPR 3)/ 16, occasionally dropping a couple cms. The EPAP min of 13 is nothing you haven't experienced frequently before. Elevating PS min to 4 is consistent with your original Rx and a common starting point for bilevel at higher pressures. It's mainly for comfort, but does an effective job to reduce hypopnea, flow limits and snores. The IPAP max of 25 is after all the reason you have bilevel. Let's see if it resolves the OA, or what it takes. As you know, the machine algorithm is programmed to increase based on events and respiration, or it will stay lower. I could set my pressure to IPAP max 25, and it would stay at 16 to 17 all night. Not so much for you.

You have severe obstructive apnea, and we have tried to use positional therapy as well as the maximum capability of CPAP to resolve it. When you first joined here it was noted you might need bilevel for exactly this reason. This is nothing new. I hope we can get those events under control. As far as you feeling like you were suffocating at EPAP 13, it's possible, all this obstructive apnea is actually central. That's why in my last post, I suggested if this does not resolve your problem, we need some "smart" clinical evaluation with PSG. Remember, we did some experiments with low fixed pressure to potentially eliminate that possibility. These settings may seem aggressive, but we have done what we can on a forum to eliminate the alternative possibilities.

Using a AirFit-20 with memory foam.  Higher pressure leaks (2- major) prompted me to hang it up a bit earlier than usual.  Believe I can get on top of that.

Looks a lot better, and I'm glad you didn't bury the needle at 25 cm. Let's go with this a couple days, then if needed, we might back off on pressure support by 1 cm to see if we can ease the centrals. Glad to see this closing in on beating the OA!

I am a side sleeper using a full face mask. I have been having a lot of leaks latley so decided to
try a cervical collar. I don't care why it works but my leaks shure cleared up. Thanks for this idea.

The neck collar aligns our neck such to prevent obstructions and also stops chin tuck as we sleep. The collar also helps to keep our mouth shut thereby preventing leaks through an open mouth.

For me, the tongue trick finally sealed 'the deal'.

I'm seeing that statement REALLY is nailed to my charts.  Being a Curious George I have to wonder the reasoning there.  If I bagged the apneas from going to sleep, waiting to go back to sleep or coming-to when I awake, mine would be pretty darn good!  Possible to elaborate on the reasoning  ajack?

The awake one is pretty obvious, you aren't choking for breath, but the machine is logging all sorts of stuff and kicking up the pressure to fix what isn't there.

you may need your tidal volume chart as well as the flags. My Vt is high when I'm awake and settles down as I drift off to sleep. I can see the point of onset. It's the reverse as I wake up. Your sleep breathing is fairly constant compared to you awake breathing.

For auto-cpap, from machine data or software. You can set the min pressure 1 or 2cm below 95%. Or clinicians commonly use the maximum or 95% pressure for fixed pressure CPAP, this can also be used for min pressure.
