Chronic Care Management for sleep disorders

Chronic Care Management for sleep disorders
For United States patrons only------- Has anyone come across Chronic Care Management services for Sleep (Similar to Livongo for diabetes) ? Does your DME offer any "coaching" or "Assistance" in getting used to your new life with CPAP machine? or does the relationship with DME is strictly for the machine and supplies? Recently completed a sleep study, most likely to get a CPAP prescription, trying to understand lay of the land with Insurance - DME - Home health services and other healthcare players in the CPAP arena. Any help will be greatly appreciated......

RE: Chronic Care Management for sleep disorders
Even from the better of the DMEs that I've used, I've not found coaching, but I've found a few people who can help answer questions I might have. But nothing has matched the advice and support I've received on this board.

No, very little coaching from these DMEs so far. They're basically Pez dispensers for PAP equipment. The best way to find a DME that you accept is to call your insurance Customer Service and get a list for about 6 DMEs near you that are in-network. Get the names, addresses, phone and website if possible. Research what others say and call each to get a feel for how they'll treat you. I don't know what level of PAP you'll need but you want to accept ResMed as they are better for therapy and comfort for most apnea patients. If the DME you call says that Respironics is identical, and that they only issue Respironics, try the next one on the list. These are the same as in both are PAP machines, but like cars are not all the same quality, neither are PAPs.
Mask Primer

Positional Apnea

Attach OSCAR, etc.
