What do you think of my first night with Resmed S11?

I am new here and new with Oscar.

I am attaching the captures of my first night with my new ResMed S11.

What do you think ?

Not bad for a first night. I think we can fine-tune things to get it even better. Good job on the chart formatting!

Also, please do not zoom in unless requested or you think there is something specific we need to see. Always post the entire night first, then zoomed in areas.

Try the following:

Min pressure 7
Max pressure 14
EPR 2 full-time
No ramp

That should be more comfortable and get rid of those pesky pressure swings. Give it a try for a few nights and let us know how it goes.

It will be important to reduce the mask leaks in order to maximize therapy. Really work on that: it takes a while for everyone to figure this out. There's a lot of trial and error, and mask adjustments are finicky and individual.

Take a look at your pressure graph. You see how it swings up and then down and up and down? I call it the tollercoaster because it takes a toll on our sleep quality. It should be closer to a flat line with maybe some slight increase and then slight decrease.

Whilst you do have some leaks they are not really significant. Something to keep an eye on, but don’t obsess at this point.

there is my second night with cpap :

i havent change my pressions :  4-12
i have change EPR to  :  EPR 2 full-time.
and NO RAMP.

is there any good changes ?

A bit better. A few areas where the flow rate looks smoother. Still quite a few leaks and pressure swings. 

You need more pressure. Try to work your way up to the recommended settings by increasing a bit night over night. Or just jump there, it is really not much more than you use today, just will feel a bit strange at first. 

You will quickly adapt to it as you give it time and relax. Many of our issues with aerophagia are related to fighting the machine rather than breathing with it. You probably noticed when you start the therapy it feels far different than it does after 10-15 minutes or so when we have relaxed.

Based on the shape of the flow rate with arousals, the pressure swings showing the tollercoaster ride of pressure swings as the machine responds to apnoeas and settles back down, and the snoring being registered. It tells me there is not enough pressure to help smooth things out.

I suggest trying something like

Min pressure 7
Max pressure 12
EPR 2 full-time
No ramp

It is only a bit more pressure than you use today, but sometimes that is all we need to splint the airway open.

You could also try a fixed pressure in APAP or CPAP mode to see which you prefer. We usually start with a bit more pressure than in an auto-adjust mode.

Mode APAP
Min pressure 8
Max pressure 8
EPR 2 full-time

Or 

Mode CPAP
Pressure 8
EPR 2 full-time

Some find a fixed pressure setting more comfortable. I use it myself as I don’t like pressure changes that interrupt my sleep. Like everything CPAP, use what works, discard what doesn’t. Just an alternative way of managing things.
