AHI looks good, but still feeling bad

RE: AHI looks good, but still feeling bad
churras, best of luck to you in your similar journey. I haven't heard of EERS and haven't tried it. Good to know about it.

Dave, attached is the ~2 hours from last night (I honestly don't know if I was asleep for any of this time). Apologies for the screen scaling.

now those are some fine looking statistics. However you didn't sleep that long.

since most apnea events happen during rem sleep i wonder if it will rise when you sleep longer.

how big of a difference is it to breathe with the ASV compared to a cpap ?

RE: AHI looks good, but still feeling bad
Admitted your AHI is great, but I'm getting the feeling it's not comfortable.

I think the exhale struggle issue is high PS Min 6, at least in part. This means every inhale is required to jump 6 at a minimum.

I would suggest you reconsider the settings. Note PS must have a spread of 5, as you have here. But I think it's starting too high and maybe ends too soon.

What I'd like you to think on:

Keep ASV Auto mode

EPAP min between 5-7, 4 will starve most adults, I think it's a setup to fail before you begin because you're air starved. EPAP Max can be about 4-5 above EPAP min.

PS you will not want to jump +6 like this with PS Min 6. Try PS Min 3. PS Max can be as low as 8 to meet the spread of 5, but it may hinder ASV therapy for the CA. Mine did blow to 25 IPAP at times, but it was brief.

Example on ASV Auto:

EPAP 5-10
PS 3-10
IPAP 8-20

You may want to plug some setup similar to this. Your low EPAP pressures and short range forced PS to make up for it frequently. Yes, PS is always added on inhale, but your min PS 6 jumped that amount every breath, closer to ST you just left.

RE: AHI looks good, but still feeling bad
Dave, thanks as always. I clearly didn't know what I was doing, haha. PS on 3.0 min feels a lot nicer. I've dialed in these settings:

Min EPAP 5.0
Max EPAP 10.0
Min PS 3.0
Max PS 10.0

My main concern is the max exhalation pressure of 10, since I have pretty significant difficulty exhaling at anything above 5, which I believe is what triggers my aerophagia. But I'll give it a try.

Take careful notes on what you feel when you use the ASV. We can help translate the feel to edits to get this working better.
