Huge leaks after hours of low leaks

Huge leaks after hours of low leaks
Hi all,

Being driven slowly insane by this. I'll have a great seal when laying down, then wake up to the machine reporting a leak rate of 40+L/Min.

When I check the data in OSCAR, I can see that I have a perfectly fine seal for the first few hours, but then something changes and it all goes downhill. Whatever's happening isn't waking me up, so I have no idea what's going on and no idea how to fix it. Anyone else deal with this? Suggested solutions?

What mask are you using? this looks like mouth breathing to me so mask seal probably isn't the issue.

It's the AirTouch F20, so definitely not a mouth leak issue.

your pressure graph looks like a mountain range.  I would try EPR full time, EPR 3 for a night and see if that helps keep the seal.

The leak rate isn't the greatest before large leaks. Median is about 15, while ResMed leak redline is 24.

I've used a ResMed F20 for about a year with I started ASV in 2017. It never controlled leaks well for me. I'd consider getting a different mask.

Myself, I used a Fisher and Paykel Simplus, as it was used during a sleep study. It worked great with minimal strap adjusting. Later, the Vitera was released and I got it. Even more comfortable. Now I'm back on therapy with the Evora Full, a hybrid. Again minimal leaks and drastically better comfort than the F20 could ever hope to give.
Mask Primer

Positional Apnea

Attach OSCAR, etc.

The median leak rate is high _because of_ the large leaks -- before the large leak starts, the median (just eyeballing) looks to be 7 or 8.

I had awful CA events until I disabled EPR entirely, so I'm disinclined to turn it back on.

Regarding the Pressure Mountain Range, note the scale on the graph -- it's moving around between 8 and 9, not between 0 and 20 or whatever.

Note that you can still mouth leak with full face masks. Even with an F20 you can drop your lower jaw enough to drop below the mask.

You can argue the median is high due to leaks. OK that's true. It's also true that prior to large leaks, the leak rate wasn't great. Leaks are not well controlled here.

that's simply not true unfortunately. your jaw can easily drop below the ffm.
