fudo - CPAP Therapy Assistance
Hello,

I have sleep apnea for 21 years.  I am now 67. Never was on any forum.

Currently using Resmed Airsense 10 pressure 9. 

Mask is Mirage activa.

I am side sleeper. Using chin strap.

I am worried about a number of CAs and H (hipoapneas)

I am kind of tired every day afternoon.

Could you guys check my therapy?  Any suggestion is very appreciated.

Have you used the EPR setting?  You have high flow limits and we use EPR to treat these.  Flow limits are apnea also but are not long enough to be counted in the AHI (10 seconds or longer).  i have how they rate apnea as O, H or FL in my signature.

I would try the following setting changes

EPR full time
EPR 3

Post the next nights sleep like you did.  You will only need to post the first set of charts.  I have a link organizing charts in my signature.
Apnea (80-100%) 10 seconds, Hypopnea (50-80%) 10 seconds, Flow Limits (0-50%) not timed  Cervical Collar - Dealing w DME - Chart Organizing

Interpret Oscar chart please
Hello,

Your pressure at 9.0 looks okay, but you have very high flow limitations resulting in the hypopnea and other events. You will need to turn on EPR full-time at setting 3 and increase pressure to 10.0. This will give you pressures of 10/7 (inhale exhale) and greatly relieve the flow limitation from which your hypopnea and RERA arise. You should notice a significant difference.

I did turn on EPR full-time at setting 3 and increased pressure to 10.0 last night.

Here is my chart.  I believe it is better now.

Do you have more suggestions?

Flow limits went from nearly 0.2 to 0.09, and events are halved. You got a little more tidal volume. This looks pretty good. You may want to experiment with higher pressure of 11.0, before deciding where to keep your settings. That should be a qualitative check (better or worse), and just go with what makes you feel best. You have some work to do on leaks. Nice progress.
