Mask for High Pressure on ASV
I'm back to my old source of the best CPAP advice around. Recently I have been forced into back sleeping by orthopedic issues
I flipped into CA and C-S breathing. Now I have an ASV and really high pressures. So how to deal with chin tuck and mask leaks.
I'm using a cervical collar, a mask liner, Air touch and air fit F20. My pressure goes even as high as 25. My sleep doc is to worried about
her work/life balance to really work with me so I'm back to my buddies who never let me down. Is anyone on this forum using the
Lena full face by lowenstein in germany? It is double lipped and made for people with super high pressures and ventilators. It's not sold
here and nobody ships it here.

Pam, we have see high pressures with ASV driven by positional apnea, so we want to rule that out first and address it if it is present. The Soft Cervical Collar and Positional Apnea wiki articles (linked in my signature) both show the effect of positional apnea with ASV therapy. We can resolve the UA events with much lower pressure, and the problem is not likely the mask. Please consider updating your Sleepyhead to
OSCAR and posting some charts showing the problem so we can get you on the path to lower pressure and more comfort.

FWIW I used regular CPAP masks on the ASV and with adjusting it correct, the Fisher and Paykel Vitera worked up to my max IPAP 25. Was it leak free? No, but low enough to not disturb me or interfere with therapy.

I think my issue is driven by the back sleeping. I have always been a side sleeper. So for the past year I have been on my back and having ever increasing AHIs and broken sleep. I had a horrible rotator cuff surgery and my sleep doc kept increasing my pressure. Well if fat rolls, goiter and
chin tuck are not squeezing the old air tube practically flat, I'd be surprised. I had a horrible sleep study recently and the doc put me on ASV which feels better and blows high pressures up to 25 when my throat narrows from a lot of hypopneas. As this happens I can see flow limit go up to 1
the ASV has cleared up the centrals and CS breathing but I think I need to wear the soft collar and work on getting back to side sleeping as I become more fully healed from the procedures. With the neck collar on I start blowing raspberries so when my nasal passages are really clear which is rarely these days I will tape but with my nose issues it's not a good thing. Even with a pressure of 25 my air way is pretty narrow because of my anatomy
and I still dont breathe well on my back. So I turned off ramp and have my pressures at 10 0 15. However with central sleep apnea I need the ASV.
I think the centrals started with her increasing pressures but I don't get enough air flow on my back so the machine does hav
e to push when I stop breathing. The pressures do come way down with the ASV and it will give me a blast of air to get me going and drop off when my breathing is better I downloaded oscar for linux/chrome book but now ResMed is not putting anymore files on my SD card.

Any chance you might share a chart or two showing your therapy results? I'm guessing most of your problem is positional, and it's not going to be a difficult fix. Clean up your SD card or buy a new one. A 4 to 32 GB SD card is inexpensive and you might as well see what is going on. It will be easier to help you if we can too. My bet is we can get your pressure at a lower level and make your therapy more effective.

Amazing!!!! Thank you a million. I have worn a cervical collar for two nights and my AHI was 0.38 and 1.0 on Thursday and Friday nights. I can't upload from Oscar until I figure out how to either wipe my RESMED SD card or buy one. I corrupted my files trying to upload to Sleep HQ.

It's great to hear you have made such rapid progress using the soft cervical collar. Once you are able to produce a chart, we will take a look at pressure settings.

Wouldn't using a computer to format the SD card to FAT32 get the SD usable again? Unless it's physically damaged that is.
