Bipap settings help? UARS and not sure what I'm doing
Hi I'm 35 and have UARS very severely impacting me. I have a high narrow palate and recessed jaws. Any ideas how much to adjust a bipap? The dumb doctor of course has set it at min epap 4 and max ipap 20 at the highest, with ps of 5. I couldn't fall asleep with it on at these pressure but just wearing it seemed to help calm my brain down and cleared out my nose yay! However I had the worst headache ever during the day and terrible GERD, which I've never had before. Any thoughts? I have a nose mask and mouth tape.  Thanks so much

Some questions…
  1. What bilevel do you have? For example, ResMed AirCurve 10 Vauto?
  2. What are your current settings? 
  3. Do you have a sleep study you can post? If so you should redact your personal data before posting.
  4. Can you download and install OSCAR? There is a link in my signature to the instructions.
Once you have OSCAR loaded, make sure there is an SD card in your CPAP and follow the OSCAR instructions to load it into the program. Once you have the data in OSCAR you can post screenshots to the forum.

We use data to help. Once we have more information we can offer advise and settings to consider.

Ok thanks. Yes I have a resmed aircurve 10.settings to 4 at lowest, 20 highest. Ps 5. Couldn't sleep in the sleep study but I'll try to post. Just showed typical UARS stuff, heart rate all over the place, many arousals. I got a watchpat so I'll be doing that soon. Bipap is just a trial from shoppers so I'm not sure I can do Oscar or change it? Sleep doctor said it was most likely not going to work for me, given my very small airway, recessed narrow jaws and terrible jaw pain from clenching. I had a DISE sleep endoscopy done and am in the process of getting ready for expansion and jaw surgery, just so sleep deprived I was hoping bipap might give me some relief while waiting.

You can change whatever you like in the machine, it is your therapy. Whether you want to or not is your decision. A good adage to remember is when you go to a surgeon for advise, never be surprised when the answer involves surgery.

We can help tweak the machine, but only if you are willing to change the settings. If you are willing to do so use the following.

EPAP min 5
IPAP max 12
PS 4
No ramp

Be sure to download OSCAR as well so you can post charts.

If you decide to wait for the surgery, that is your choice. You can always circle back here afterwards if needed.

Thank you so much for that help!! I really appreciate it. I have no idea how to change the settings on it, but I will play around with it. Is no ramp a setting I can adjust as well? Is there a full face mask that doesn't push on the jaw that people tend to like better? I really don't like the mouth taping, my skin is just hating the tape and has gotten really sore, if you have any suggestions for that as well.

I'm trying to attach my sleep study but it keeps saying that file is not allowed? hmm

There is a new mask out from ResMed, called the F40.  This is a hybrid full face mask that covers mouth and sits just under the nose.  Folk here seem to like it.

Ok thanks, I have no idea how to break down a file like that? It's only 6 pgs or something. Not tech savvy and so exhausted I can't focus to figure it out. Thanks about the mask, I also have the Philips under nose face one as well, may try that again. Unfortunately so tired and cognitively challenged, I can't really read anything
