New CPAP User could use some advice
           

For the last 5 years have felt tired all the time, never suspected sleep apnea. Got a lofta home test which came back at 33 AHI, all obstructive. 33 AHI Left side, 2.7 AHI Right, 43 Supine (Approx numbers). 

Had maxomandibular advancement performed almost 2 months ago. No noticeable change in breathing or sleep quality. I had a OK nights sleep the previous night with CPAP, and then last night was awful. I wear a garmin watch and can see my stress levels throughout the night which closely correlates to how I feel in the morning. I had as bad a nights sleep last night as if I had not worn the CPAP.

Using Resmed 10 Autoset

I understand these things can take time, however I do know my body is a quick responder to when, whatever the issue is, is fixed, as 3-4 times over the last few years I woke up feeling so well rested, it was the most amazing blissful feeling, better than any drug. I tried to replicate that in every way I knew how, with no luck. 

I'm assuming I have some settings off here. Any advice would be so greatly appreciated. Thank you!

I would try setting the EPR at 2 full time with 9cm minimum pressure and max pressure at 16cm to start.

Having the machine in full time EPR should lower your flow limits and make your sleep more comfortable.  It will also make it easier to determine what your pressures should really be set at.

As to the ramp, turn it off if at all possible, you are getting little if any benefit when it is turned on.

You are having some major leaks that the CPAP machine can not compensate for that you need to address. During the major leaks (+24) you are not getting proper treatment if any.

Try these setting for a few days then post some more charts (see below for info on chart organization)

Next time you post, could you fit all of the following into a single screen shot (and include nothing else):

Left panel
Events
Flow rate
Pressure
Leaks
Flow limitations
Snores.

That will make it easier to see what's going on.

I agree with the suggestions Steve has made for revising your settings.

Thank you everyone. I am honestly very lost here. I did do similar settings to what was advised above and didnt really feel much improvement. I have had a few good nights sleep randomly so I have learned to trust the data I get from my garmin watch (HRV, stress levels while asleep, respiration rate, heart rate) and it does correlate with how I subjectively feel.

I have noticed alot of weird breathing patterns on the flow rates as well. TBH i am very sleep deprived, sleep doctors aren't helping much, I have a very big project that I'm falling behind on I could really use some help in depth looking at past charts, I can post the zip file here (I managed to lose alot of data somehow in the transfer which is unfortunate). If anyone who has experience with UARS and troubleshooting etc I would be happy to pay for their time, or buy you a gift card, send you flowers, whatever is appropriate, it would at least give me a whole lot more information to go on.

frankie - Please read the links in my signature for the proper way to display a Daily screenshot using the F12 (Fn+F12 for a Mac).  Reading other member's therapy threads will help demonstrate what is needed.

Frankie, it'd be great if you could just post the Oscar chart for the night (or two?) that you used the revised settings.

Hi Dormeo, I had started out with those settings, and moved it up to 11-16 Cm2 when I saw the pressure was reaching those levels (12-13 average) and staying there. I had EPR on initially, but turned it off when I started noticing some Centrals. However I did not give myself time to let the centrals clear out, so since then I have turned the EPR back up to 3. Regardless I was still sleeping badly (subjectively and recording RHR, HRV, respiration, and a few other general indicators)

Unfortunately I managed to somehow lose the previous data on OSCAR when I was trying to send files to a doctor. So now I only have several days. I will def keep EPR on, but it does seem like going down the same previous road to me as thats where I started. However if that data is needed for graphs, I will definitely do so. (Sorry Im still learning here)

I included 3 random screenshots from several days ago of flow rates when no flow limitations were showing. I am really new to this, but I am showing very low AHI, but I am i in the unusual position of knowing that 3-4 hrs of good sleep, and how I feel with that, as it has somehow happened to me several times over the last 5 years, and I could see the data on watch to confirm so it wasnt only subjective, so I am fairly certain this is not a "wait and in time it will get better" kind of scenario. My RHR, respiration rate etc all being far higher at night, and HRV plummeting during sleep (it should do the oppposite), regardless of the low AHI my body is panicking during the night. 

I suspect UARS but the only thing I have to base this on is knowing I have severe sleep apnea, knowing that my body responds almost immediately when whatever is needed, is done, and knowing that its happening in my sleep, and knowing I have a low AHI with cpap (and very often without). 

Any help would be so greatly appreciated. Thanks everyone

All of your setting in the left column shows EPR is set to ramp only... It should be set to Full time.
Apnea (80-100%) 10 seconds, Hypopnea (50-80%) 10 seconds, Flow Limits (0-50%) not timed  Cervical Collar - Dealing w DME - Chart Organizing
