Turned up PS to 5.0 and p95 flow limitations are now 0.0 for last night, pretty nice to see this work out so well for her!

Two weeks later my wife is saying she still feels very tired / unchanged, even though flow limitations are ~gone now.

One thing I noticed is that her median TV and MV very high now - is she over-ventilated? Could that be causing fatigue?

She is 5'7'', according to this table her ideal tidal volume (6-8ml/kg) is 370-490ml, but she's now at 560-600ml:
