New User 1 week, still high AHis, thoughts?
Hi,

I'm new to the forum and the APAP / CPAP sleep apnea world.

Been trying to interpret the Oscar results.  Any help is appreciated.

I have been averaging 12 - 16 ahi and not sure why they won't reduce since I started treatment

Sleeping better through the night but still waking with bad headaches and tiredness.

If these are the wrong graphs let me know

Thanks,

Ron

Not needed today but next time. Please don't zoom the OSCAR chart unless requested. Overall view is a better starting point.

Your Apnea is still fairly high, and I think will be addressed with a few edits to pressure. Try the following:

Turn off the Ramp - it can negatively affect CA, it also reduces your treatment time every time you start the device not just per night but per restart of CPAP

Increase Minimum pressure to 8 or 9, 9 is just over your median pressure, more pressure should reduce events, also increasing equal to or over min 7 allows EPR 3 to work fully immediately upon therapy start

The Hypopnea indicates needing more pressure. Hypopnea are partially restricted Apnea, and seem to be your highest target to address now.

Leaks are somewhat a need to address as well. They're somewhat higher than desired.
Mask Primer

Positional Apnea

Attach OSCAR, etc.

I believe You are having positional apnea.  You can see positional apnea where either H or Oa events are clustered together.  Getting rid of as many as you can will lower your AHI.  Positional apnea can NOT be controlled by pressure changes.  You have to find out what position you are getting into and cutting off your own airway.  Have you changed your sleep position?  Sleeping on your back?  Using more (or new) pillows?  These things can cause positional apnea by chin dropping to your sternum and cutting your airway.  Think of it of a kinked hose – nothing can get through – you have to unkink the hose…

IF you can’t make a simple change like changing to a flatter pillow or sleeping on your side helps then you will need a collar.  I have a link to collars in my signature at the bottom of the page.  It shows people who are not wearing a collar and the SAME person wearing a collar.  There is a huge difference between the two.

Thank you.

When you say too many leaks, what do you refer to?  I was unable to tell if I was having too many?  My mask scores seem to be pretty good I thought.

Can you say what is normal?

Thanks

Ron

I usually lay on my left side but not sure what else I can do, I guess i can try a new pillow maybe or less pillows

ResMed has a leak redline at 24 L/Min. Yours aren't as high as that, with 95% at 13.2, so that's not terrible. However the summary graph has you at 23 or so 95% last week. The closer your leaks get to 24, the more likely therapy will be hindered by inaccurate machine reading and it'll likely be more disruptive.
