SleepyEvan - Therapy Thread
Hi all. 42, M, 5'10", 165lb
I was recently diagnosed with severe obstructive sleep apnea. The results from my home sleep study were an overall AHI 0f 68.8, almost all OA events (66.2/hr), no CA and some H events.
My doctor prescribed APAP therapy so I purchased a Resmed AirSense 10 Autoset. I'm using the Resmend P30i nasal pillow mask and heated tubing. I'm not a mouth breather, so the nasal pillow seemed like the least intrusive option.
I've been using the device for a week now and tolerating it okay. It's a bit tough to get to sleep as the device makes me very focused on my breathing. I am waking up a few times during the night and getting back to sleep can be challenging. My AHI on the machine is quite good, well under 1 and I do feel more rested and less foggy, so I'm happy for that. I seem to get a lot of flow limit events though and the pressure goes up and down all night.
My first four nights I had no OA's and only a couple of H or CA events per night. After playing around with Oscar and doing lots of reading on the wiki I realized that the supplier hadn't set the machine with my prescription, which is 4-15cm H2O, EPR=2, 30 min ramp. The machine was set to 4-20, EPR off and auto ramp. I changed the settings to those from the prescription hoping the EPR would help with the flow limits. I've kept those pressure and EPR setting for three nights and have had 2-5 OA events per night. I've also been experiencing a bit of aeorphagia when I wake up since I made that settings change. The 30 minute ramp didn't work great for me in the situations where I woke up and needed to restart a session so the pressure would go back down to min so I could get to sleep. I had trouble falling asleep with ramp off, so I switched ramp back to auto.
I think I'm going to turn EPR back of for tonight, to see if the OA's go away. Any advise on changes that might improve sleep quality and reduce the flow limits would be appreciated.
