[CPAP] Requesting Feedback & Suggestions based on OSCAR result

Requesting Feedback & Suggestions based on OSCAR result
Hello, I'm a newbie to this forum. I recently learnt about OSCAR and how to extract from my CPAP machine. Want to share my last night's result to get feedback & suggestions for improvement.

Context:
I'm on my thirties, BMI in 26ish, had borderline hypertension & prediabetic last 4 years and I had been observing poor sleep quality starting with pandemic times. I thought it was due to increased screen time and poor sleep routine. Tried to understand science behind sleep starting last year and tried for months to improve on it and tracked via AppleWatch. Even if I sleep 7-8 hours, I clock <15 mins of deep sleep. Also my partner observed once that I was very active rolling during the sleep and few times grasping for breath. I reached to my PCP and got sleep study ordered. My study results were as follows:

SLEEP OPPORTUNITY: Total recording time (TRT) 527 minutes, Total sleep time (TST) 501 minutes,
sleep latency 4 minutes, wake after sleep onset (WASO) 17 minutes.
RESPIRATORY: There were 275 obstructive events, and 11 central events for a sAHI of 34.3 per hour
sRDI 36.1 per hour. The oxygen saturation <88% was 1 min.
IMPRESSION:
Severe obstructive sleep apnea

I got my CPAP (AirSense 11) and using AirFit P30i Nasal Pillow mask (after trying couple of different masks) for last 2 months. My average AHI last two months is 2.65.

My last night session's OSCAR snapshots is attached here: https://imgur.com/a/cWJsKU9.

Please review to provide feedback and suggest how I can improve further?

Thanks for your time!

Welcome to Apnea Board,

The settings are giving decent therapy, leaks seem minimal and controlled.

A few things that may help improve your sleep, I'd try the combo of minimum 7 instead of 6, and follow-up with making EPR 3 full time.

Next, I'd consider trying to get off the Ramp, as it's possibly a distraction and keeps you out of therapy for its duration. If you look at the pressure line on the chart, you can see that flat hold it had.

Also you can post directly at the bottom of the text box where it says New Attachment. Look in my signature about attachment directions.

Hope all goes well and check back.
Mask Primer

Positional Apnea

Attach OSCAR, etc.

Thank you for the feedback and suggestions! I got the machine via the insurance and I read something about policy compliance. I will clarify that before making the suggested changes! Yes, I tried attaching the PDF export from OSCAR app but it did not let me do it. I will check the instructions again and share via attachments next time.

Compliance involving insurance means using the machine 4 hours per night, 21 of 30 nights, with 90 days to meet the 21 days of 4 hours out of 30.

Typical will be Medicare guidelines that most others follow too.

There's nothing that requires you to leave the settings alone, that you can't edit. It's only time based usage.

You also have a doctor follow-up within 90 days of machine issuance.

Thanks, yes looks like what you have explained. OK, will explore the settings and share the results in next few days. Appreciate your time and help.

OSCAR screenshot shouldn't be a PDF. It'll be a data file that you can use the New Attachment area.

Quick follow up question on this part. Which part of the flat line you are referring to the ramp time?

Per my understanding, the flat line has two parts: I presume, the X axis unit is 3 mins per bar. So about 9 to 10 mins, the pressure is 5.0. I can confirm this is the starting pressure when I hook the hose to my mask. Per chart, after 10 mins and up to the point the first OA event occurred around 1 hour into sleep, the pressure was flat at 6.0 which is my minimum baseline. So I think the ramp time is just 10 mins. Are you suggesting to take this off? Thanks.

Right, the ramp is only about 10 mins at 5.0 cmH20. Rest of the period is at 6.0 which is the low end of my configured range. Since no events occurred until 01:07:40 am, the pressure remained at 6.0.

Today/Tomorrow I'm going to tweak the suggested settings and share the results.
