[Treatment] First Night

First Night
I finally was able to unbox my Resmed Airsense 11 and put it to use for awhile last night.  I started the evening wearing it in bed watching TV.  I felt like the nasal pillow was comfortable, I didn't feel like I was suffocating until I fell asleep.  Every time I fell asleep and the machine increased the air pressure to 8 and above I woke up feeling like I couldn't breathe. I checked the pressure and it was in the 8s each time I woke up feeling like the pressure was overwhelming.  I ended up taking it off after 1.5 hours.  Unfortunately, after I removed it I literally realized that I was actually struggling to breathe with it off.  Is that normal?

Settings used: 
Mode APAP
Min pressure 6
Max pressure 12
EPR 2 full-time
No ramp

I have an extremely petite facial structure, but larger nostrils for the size of my nose structure.  I am going to try the medium pillow size tonight to see if that makes a difference.  Any advice based on this very small snippet of OSCAR data?  Would it be worth while to decrease the max pressure to 8 or 7.5 and see if that helps the situation?

Welcome, and congratulations on starting PAP therapy!

Today, please set your machine up *outside* your bedroom during the day or evening and try using it for a couple of hours while your read, watch TV, or use your devices. This will speed up the process of getting used to the new experience.

You may be experiencing some sleep/wake transition glitches. These are likely to go away as your body gets used to the pressurized air at night.

Do you experience nasal congestion? I ask because (judging from the left panel) it seems you are having a fair amount of flow limitation, which might be (or might not be) messing with your sleep. If the problem is in your nose, you need to treat the congestion (e.g., with Flonase, which takes a week or two to kick in). But if the problem is further down in your airway, a little more EPR might help.

During this first week, as you're trying to get used to everything, I'd suggest you set your min and max both to 7 and your EPR to 3. Let us know how that goes.

Thank you! I always struggle to breathe out of my nose, but my ENT swears it can't be congestion. I had bilateral sinuplasty and he said he really opened up my sinuses so I should not be able to really get a sinus infection ever again and that it should drain effectively. Today I had a runny nose, upset stomach, and major headache but that may have been from the CPAP. I will try these suggestions out for a few days. I have rearranged my OSCAR graphs so they will be in the right order next time. I was thinking from my research maybe it would be good to just try one level and see how that goes. I know that I couldn't breathe well even with the CPAP on my back last night. So I really tried to get myself situated on my side. The info from Lofta said to try to fall asleep with it for a week but not force it right away, so I am trying to hold on to hope. It stinks when you are actually sleeping worse than you were before. Big Grin
