Members have tried a lot of alternative approaches to resolving the positional apnea issue, because the idea of SCC is not very appealing, but the results are in, and it works best. Most people that need it get comfortable with it quickly, and some even train themselves to avoid positional obstruction. The choice is pretty simple, and changing CPAP settings is not one of them. No amount of pressure will push through the positional obstruction. Your friend's pressure will stabilize at a much lower level and he will get better sleep once he addresses this problem.

Your friend has clusters of Obstructives that appear to be positional apnea. Have him read the articles That SleepRider listed, which may help him understand why a "soft" cervical collar could help (or any other method that would keep him from tucking his chin into his chest). Has he actually tried one? The correct fit, circumference and height is important.
