No, not really. Some things are obvious, for instance when you disconnect to go to the bathroom, there's a big gap. But if you held your breath for like 20 seconds while awake, the machine would not be able to distinguish it from a real apnea.

Overall, it's probably not a big deal though. Once you get used to it, you won't be spending a whole lot of time with the machine on and not asleep.

It is designed to detect breathing and the lack of breathing. It detects this by looking for, basically, "echos" in the pressure. No echo at all means the mask is off so it doesn't record anything. Different echos mean breathing fine, narrow airway, blocked airway, airway open but no breath.
