Here you go! and sleep on my back is sthe most comfortable; try side sleeping with all kinds of tricks and it's not for me.

Thanks mate.

An AHI of 2.1 whilst sleeping on your back is most impressive. Not quite as impressive as snoring 80% of the night, but still quite an achievement, just not in a good way. Unless you were purring in your sleep…

Please repost the original chart in your original post using the OSCAR chart formatting as in my signature “Formatting OSCAR Charts’. I think we can get much more data from that than from the sleep report.

I do not see anything that is concerning to me in the least. What exactly are you concerned about in the chart?

What I see is zero snoring, very few flow limitations, good I:E ratio, solid tidal volume. I would not change anything. The CPAP is doing its job quite well.

Clarify/ Increasing Epap for snoring /arousal ?
    attached is a night after bipap I still feel sleepy. If 99.5% epap is 5.6 should I increase it to this. epap is currently 5.

Just to gain clarity if I want to limit flow restriction as some one with mild sleep apena / uars will my new setting be 

5.6-6 epap 
4.4-4.8 PS
10-10.4 Ipap

Any issues with this ?

Accumulation of mucus causes arousals?
here is my endoscopy video (click here)

Question: is the accumulation mucus the cause of my arousals?
Issues:
  1. Obstruction:
    • Accumulation of mucus can lead to partial or complete obstruction of the airway, making it difficult to breathe, especially during sleep. This can cause frequent awakenings and disruptions in sleep.
  2. Irritation and Coughing:
    • Mucus accumulation can irritate the airway, leading to coughing and discomfort, which can further disturb sleep.
  3. Post-nasal Drip:
    • Excessive mucus can cause post-nasal drip, leading to throat irritation and a constant need to clear the throat.

1) Is this conclusion (see bottom) part true about epap and ps in terms of prevent CA?
Comparison of Settings
Option 1: Increased PS to 4.4 cmH2O and EPAP to 5.6 cmH2O

PS: 4.4 cmH2O
EPAP: 5.6 cmH2O
IPAP: 10 cmH2O (PS + EPAP)
Risk: Slightly higher risk of central apneas due to increased PS, which might cause hyperventilation.
Option 2: Increased EPAP to 6 cmH2O and PS at 4 cmH2O

PS: 4 cmH2O
EPAP: 6 cmH2O
IPAP: 10 cmH2O (PS + EPAP)
Risk: Lower risk of central apneas as PS is moderate, and EPAP is increased to keep the airway open.
Conclusion
Option 2 (Increasing EPAP to 6 cmH2O and keeping PS at 4 cmH2O) is likely better for preventing central apneas. This approach maintains a balance between preventing obstructive events with adequate EPAP and minimizing the risk of central apneas by avoiding excessive PS.
