Feel worse on lower EPR even though AHI seems the same? - LarkinM - 03-12-2024

Why does it seem like I get worse quality sleep with EPR 3 vs with 2 even though my AHI looks about the same? and my leak rate looks better on EPR 3. 

I feel way worse on EPR 3 and have tried it multiple times. 
Any other settings advice is appreciated.

I would raise the min to 8 EPR full time set to 3.  No other changes.  Lets see if  that helps.

When I was using an AirSense 10 Autoset, I usually had a lower AHI when EPR was set to 1 rather than 2 or 3. I suspect the reason is because EPAP controls apneas while IPAP controls hypopneas. So when you increase EPR it decreases your EPAP. Bipaps work opposite, since Pressure Support increases IPAP over EPAP.

Tried the settings you recommened. Felt pretty sleepy and it gave me some pretty bad stomach pain I've felt all day. I didn't change anything else. Any suggestions? Lower pressure? Different EPR?

What settings have you tried prior to this? It seems you've got several threads going on.

Your events look low, seeming to be decently controlled by this lower pressure set.

7-10 epr 3
8-10 epr 3
9 epr epr 2
7 epr 2 
8 epr 2 
5 - 10 epr 2
not sure what to do bcs of the stomach issues.

Which, if any, did you feel best with? Maybe you can try a pressure without range but still with EPR. Maybe try 7, 8, 9 only and EPR 3. Either use regular AutoSet mode and put that number in both Min and Max, or CPAP mode with that number and EPR 3.

I was on 7-10 epr 2 for the longest. 80 day streak and then started messing with my settings and haven't found something consistent. 

I will get stomach pain from the cpap and that will make me want to change my settings or sleep poorly and not be sure what to do.

Is there some stomach issue that could be a hindrance? If there is, maybe we can help with it.

Myself I have hiatal hernia, acid reflux. I had to build up tolerance to CPAP pressures, and my last was at a highest of 25 cmH2O inhale. I sleep slightly inclined and mostly on my left side. Just don't stack lots of pillows or it can make Apnea worse.

Yeah definitely have some acid reflux. I take some over the counter medication for that. I also have tried keeping track of dietary things that trigger it. 

I wonder if that's the real root cause of sleep being worse at random times. 

What do you mean by don't stack pillows?
