RE: VAuto Journey - Sleeprider - 07-15-2020 We are getting caught between a rock and a hard place with these results because it is a mixed signal of obstructive and central. This simply doesn't fit conventional titration protocols and moves us closer to ASV. To limit obstructive events and minimize pressure variations, let's increase EPAP min to 9.0, and we are going to cap it at EPAP max 10.0 to prevent CA. You are at PS 3.0 and I don't think we can increase that a lot without increasing the CA, but we want to provide support for hypopnea. Let's set PS to 3.2 and see how it goes. All of this points to more of a central problem than obstructive. Just start taking it up to 3.6 then 4.0 as needed and tolerated. It's not bad, but I do notice I feel a little better when it's close to 2.0 or lower. Keep increasing my PS? Or is there anything I can do? Had you tried that yet? I had it at 9 on June 9th. I had an appointment with the NP at my sleep facility and she lowered it to 7. I stayed on 7 on June 10th and 11th. I raised it back to 8 on the 12th and haven't changed it since. RE: VAuto Journey - SarcasticDave94 - 07-28-2020 It's possible a bit more EPAP may be needed. See what the others say, but I think the change from 8 to 9 referred to is still valid. |
