I havent changed anything except put the min pressure back to 7 after I was getting bothered by excess air venting into my eyes. But that's a red herring because Id been at 7 for ages anyhow. Im guessing it's mostly positional, but there's not much more I can do to improve that. I can see that the pressure goes up to my max of 12. Would it be better to increase that a bit or not? Thank you RE: Little Olive - Therapy Adventure - UnicornRider - 06-24-2024 You have not tried CPAP Mode, It could be that the change of pressure, as well as the air swept fuzzy eyeball, is contributing to your awakenings. Your Median pressure has been up there ~10 cmH2O. So try setting both Pressure Min & Max to 10.4 cmH2O, change mode to CPAP. To summarize: Mode CPAP Min pressure 10.4 Max pressure 10.4 EPR 3 full-time No ramp Smart Start OFF After a couple of days you can try changing EPR to 2 cmH2O, if CA events are still high. It does look as though the events are clustered, as in positional apnea, Soft Cervical Collar, low Pillow and side sleeping are the recommendations. Could you post an OSCAR Report with ~12 minute zoom of the start of your CSR event, include ~1minute of pre CSR in the view. Be sure to include the OSCAR Reports with the CSR events captured in your package to take to the hospital. Try those settings, post back and let me know how it felt with these settings. RE: Little Olive - Therapy Adventure - Little Olive - 06-25-2024 Good morning, I was OK with the cpap 10.4 but pretty much at the limit for me, my mask was just barely holding out and I did have to tighten the straps a bit which was also at the max I can tolerate of tightness. So I dont think I could manage a higher setting. Try again tonight. I start out on my L side, and try to stay on a side, not back, and of course have a flat pillow, but I am such a restless sleeper and always trying to be comfortable and evade pain, so I can't always control my position. It's a bit of a shambles, and not sure how this has happened after several weeks in the 2, 3 and 5 AHI ballpark. I might need a deeper profile SCC. And a whack with a sledgehammer at bedtime. Never mind the sledgehammer, I feel like Ive been in a medieval torture device all night. The super duper AHI does not translate into refreshed sleep and wellbeing! But mustn't grumble, because a few aches and pains and irritations will not kill me, whereas apneas can, so here's to the SCC. If that therapy mode and pressure is not uncomfortable, let it ride for a day or two, to see what the numbers do. You are a week out from you Hospital Sleep review, I wish they had a library where you could check out a machine to trial. We could have trialed you on an AirCurve 10 VAuto by now and figured out if that would provide the needed relief or if we would need to go back to the library and check out a machine with ASV. Don't beat yourself up too badly about the positional apnea. I go through phases where I definitely believe I am getting positional apnea, it usually occurs during allergy season, so it sometimes is hard to sort the cause of apnea out. I normally sleep on my back, but during allergy season I move around a bit. Sleep well, you are almost there. RE: Little Olive - Therapy Adventure - UnicornRider - 06-26-2024 You sneaked another reply in while I was posting the last one. Yes, if you get the right collar, it makes a big difference in AHI. Now you have to work on the comfort level. I am really glad you experimented with the SCC. I still believe that AirCurve 10 VAuto or ASV will provide better restorative sleep. Be sure to log the miserable non-restorative sleep you experienced with this AHI score, in the Notes tab of OSCAR Daily view. I wish I could tear a couple pages off the calendar. Well I will just leave things as they are for the next few days, won't hurt for the hospital to see all the uglies of the latest week. They did call me to say bring my machine in with me, so they will probably look at the data. Apparently a technician is also going to check it and service it if needed, although it's only 3 months old. RE: Little Olive - Therapy Adventure - UnicornRider - 06-28-2024 Generally, most medical reviews of CPAP data review consist of compliance, hours and days of use & AHI score. Now I will say I am blessed, as after I started taking OSCAR Reports in to my Sleep Center to reinforce my questions, They took an interest in OSCAR. A few months later at one of my compliance reviews, when I handed the nurse the OSCAR reports of issue, she spun the monitor around and showed me she was reviewing my card with OSCAR. It seems as though the software the manufactures provide to the clinics is no where near as deep as OSCAR. So for true analysis of the information they utilize OSCAR for those hard nut, tough to crack cases, like me. They even showed me how to manipulate the data in OSCAR to meet compliance. It seems as though figures don't lie, but liars can figure. Or another way to look at it, always put your best foot forward. In your case we want the worst foot forward. You will not have any compliance issues, other than failure to meet compliance for AHI. Hopefully the reason they want you to bring in your machine is to trade it for a more expensive machine that will meet your needs. Have you been watching the data populate the Overview and Statistics views in OSCAR? Maybe we will have you post a screenshot of the Overview and Statistics views before you return the AirSense 10. I always made a clone of my SD cards before I returned a machine. Hang in there, You will do well. RE: Little Olive - Therapy Adventure - Little Olive - 07-01-2024 Thanks for reminding me to screenshot the stats and data , yes l do look at those. Oscar is amazing !!! And yes, the Resmed so-called detailed data the clinic have is not a patch on Oscar, l definitely wouldnt call it detailed! So I have printed out a fist full of my ugliest Oscars to take. I still have no idea what they will say, tbh if they are working purely by AHI they will probably just send me home saying lm doing great. I certainly don't feel great. I won't accept that. Im so fatigued and just want to sleep. At my age Im certain they won't give me an ASV because l have not had a full cardiac assessment. A chest xray and an ecg are not a check for heart failure. I think that ASV is unsafe with HF so HF would need to be excluded. That would take an echocardiogram, which has not been ordered. I did have one 4 years ago which was fine for my age, but in the past 12 months l have breathlessness and more fatigue so who knows? I just hope they give me something or do something to help me because lm struggling. Thanks for all the support l have received here , it's so appreciated. RE: Little Olive - Therapy Adventure - UnicornRider - 07-01-2024 If they follow the pattern I have seen here and in Canada, they will try to go the cheapest route possible, so before they go full ASV they will go with the AirCurve 10 VAuto. As for the AHI, tell them you have exhausted all the modes and titrated the AirSense 10 to the best AHI possible with that machine and you are not getting restorative sleep. You have friends who are really happy with the features and flexibility the AirCurve 10 VAuto has to offer. I am eagerly awaiting to hear the results. May you have a safe and productive journey.
