Hi everyone,

Two weeks later my situation did not improve that much unfortunately. Even the good night I have (7-8 hours of sleep, < 2 AHI) I still wake up tired and with a headache. The headaches are maybe even more intense than before my CPAP use? I'm not sure, but it feels different.

Last night I woke up in the middle of the night with a strong headache, and a feeling of pressure around my nose (sinus pressure ?). I could not go back to sleep immediately. Usually when I wake up I have trouble getting back to sleep, I feel like I can't breathe normally with the cpap pressure. Ramping up usually helps, but 4 cmH2O is maybe a bit too low.

Does anyone have advice about this? Is there some changes I can do that would help with the headaches? I played around with the humidity, currently at 4.5/6, I don't know if I noticed a difference.

Also, if anyone has information about my previous messages, this would be gladly appreciated.

I reread the messages I got previously, as suggested tonight I will test with EPR at 2. I also found how to adjust the ramp settings too, I will try to start the ramp at 5, might be easier to breath in the first few minutes.

I forgot to mention that since I started the treatment I have aerophagia. Is this wiki page up to date regarding this? I sleep on my stomach, with my head tilted so it can rest on my pillow without disturbing my mask. I have difficulty falling asleep when I'm on my back, but I guess I could try one of the two positionnal approach suggested in the wiki.
Would lowering the pressure be advisable in my case? It looks like my pressure is often at the minimum or a bit over the set minimum.

I didn't wake up as much with the new settings. And I definitely have less discomfort with the ramp starting at 5. I'll continue to use EPR at 2 to see how I feel in the following days.

Flow limitations are more frequent, I don't know how much it is relevant though.

Everything looks good! I think you're headed in the right direction with EPR at 2.

Flow Limitation looks ok too. However, I would try to wean off the ramp or set it to
6cm for a shorter period of time.

You didn't say how the Aerophagia was last night.....

Thank you for your feedback. I will decrease the ramp duration tonight, and try to increase the pressure at the start of the ramp incrementally. I still have a bit of trouble getting to sleep if the pressure is too high, but I guess I will adjust progressively.

Regarding the aerophagia, I still had a bloated stomach this morning, maybe a little bit less. It varies from night to night so it's hard to say if my changes made a difference for this.

Does anyone know why events and the flow rate graph are not aligned properly? Pretty sure that the CA event should be 2 minutes earlier. Is this an issue with my machine? Is there a setting to adjust in OSCAR? That's not that much of an issue, just a bit weird when I try to find the relation between an event and the flow rate graph.

Anyway, following on my previous messages. I feel like I'm on the right track. Morning headaches today and yesterday are not as strong as the past week, which is great. I am still tired, but yesterday I had more than 8 hours of sleep and it didn't feel as bad as some of the day. Hopefully this trend will continue!

Regarding the aerophagia, it still varies from day to day. I feel like if I sleep on my stomach I am a bit more bloated (or maybe I notice it more). It was bothering me a bit when I woke up in the middle of the night today, hopefully i'll find something that works for me regarding this.

I attached two more screenshots with the OSCAR data from today and yesterday, if anyone is curious.

All right, small update after a few days.
  • I still have headaches when I wake up. It still feels like that the pain is from my sinuses.
  • I still have aerophagia, and I can't find a position that improve the symptoms and that would allow me to fall asleep.
  • I still feel tired when I wake up. But I am maybe a bit less sleepy during the day? If there are improvements, they are barely noticeable day to day.
I am trying to have a better sleep hygiene (eating healthier, going to sleep around the same time) too. Overall my mental health seems to be better, but I don't really know if the cpap was the cause of my improvement. It's currrently hard to say if the cpap is helping or not.

My plan would be to see a doctor to discuss about the headaches and the sleepiness, to see if there are way to help with the cpap and/or see if I might have something else that would explain these symptoms.

My questions are :
  • Do you have any advice for the headaches and sinus pain? Should I lower my pressure to help? I've also read that changing my mask could help reduce the pressure going to the sinuses.
  • Could the headaches reduce the benefits of the cpap? Is there a way to confirm that the cpap is indeed useful (I still have doubt regarding my sleep study)?
Any help is greatly appreciated. I know cpap might take time to see benefits, I might still be adjusting.

Just a thought; I see you are using a P10. Have you checked to make sure the micro diffuser material is venting properly? The mesh is so fine that moisture can easily clog it, thus preventing proper exhaust. This can cause you to rebreathe your expelled CO2.

I feel some air going out from the front when I breathe.

There has been a few time were it felt like I couldn't breathe properly in the middle of the night and I had to take my mask off for a few minutes. It does not happen as often as it used to. When it happened I did not check if air was still venting properly though.

New update. I went to the sleep clinic to try a new mask, a Resmed AirFit N20. In hope that it would help for the sinus pressure. I am still renting my machine and would soon need to make a decision to buy one or not (in a month).

I could talk with the respiratory therapist about my sleep study (that was done by another clinic) and she told me that they don't usually count the autonomic hypopneas (hypopneas without oxygen desaturation). Which seems to be in accordance with what I read and posted in my previous messages... So, I might have mild sleep apnea but unfortunately the study I did was not enough to prove it, it might be a false positive here, and without doing a sleep study in the laboratory I won't know for sure.

This is quite difficult to handle as I still am tired and waking up with headaches. I would be grateful if anyone could review my first messages and see if there's anything that could be done regarding the therapy.

I will try and see a doctor as soon as I can, hopefully testing other hypothesis. The respiratory therapist also suggested that I try to see the pulmonologist that made my diagnosis, to see if I could reduce my pressure (hoping to reduce the aerophagia and the sinus pressure), that might be a bit more difficult though, but I need to try.
