No, Your CAs will increase. Try setting your Trigger to High. You are getting close to the default Ti Max time. I would increase it to 3 seconds to make sure you don't get cutoff mid-inhale.

My trigger was already set to high for that session. I'm not familiar with the second part. Do I increase my max ti to 3 seconds? Is that under settings?

And then final question: Should I reduce PS to 3.5 and increase EPAP to 8?

There is nothing wrong in trying it. Unless you see a major change in your data on the bad side, let the change ride for at least 3 nights to make sure it is working in a positive direction.

Hey there!

Yes I do. Unfortunately I cannot post images as I've exceeded my MB limit, but I will tell you the data compared to the first night I posted.

WHAT I CHANGED ON NIGHT 2:

1. I slept on an elevated wedge pillow in a more supine position for a lot of the night.
2. I decreased PS from 4 to 3.8
3. I increased EPAP from 7 to 7.4
4. I increased Trigger setting from High to Very High.
5. I increased the T.I Max to 3 seconds.
6. IPAP remained same at 15.
7. Wore cervical collar all night this time around.

RESULTS:

1. No centrals! However, the centrals were replaced with 4 x Hypopneas
2. Leak rate went from a Med of 1.2 to 13.2 (My theory on this is the elevated wedge may have caused gravity to open my mouth more despite mouth tape??)
3. Flow Limits increased (99.5% increased from 0.11 to 0.15) and there were more of them (possibly from Supine position on elevated wedge or reduction in PS?) 

So... I am thinking I will return to my usual non-elevated side-sleeping position. Keep the trigger at very high, increase PS to 4 again. Keep EPAP at 7.4. Keep T.I Max at 3 seconds. Increase Mouth Tape yet again. 

What do you reckon? Any thoughts? Also, have you heard of elevated wedges causing worse results in some people? I thought they were supposed to help.

Glad to hear that changing the trigger sensitivity helped eliminate central apneas and you've only got hypopneas and likely RERAs (not scored on Aircurve 10 VAUTO). How did you feel after sleeping this morning? Better than normal? Worse? Only having 4 events all night is pretty great.


Based upon your posts I think it would be helpful to cover some bilevel basics:

1. EPAP should be the pressure that eliminates obstructive events (light blue) in your case 7.4
2. IPAP is used to control hypopneas, RERA and snoring 3.8
3. PS or Pressure support is the difference in pressure between IPAP and EPAP. 4cmH20 is a typical starting PS for bilevel users

Effectively, you're settings are 11.2 / 7.4, PS 3.8 and tonight it sounds like you'll be running 11.4 / 7.4, PS 4 - why did you decide to increase IPAP?

Why are you making such incremental changes? What are you trying to avoid? Aerophagia?

I sleep on a wedge pillow & a full face resmed f20 myself and have been able to maintain a 7 day leak avg of 0.02. The most likely culprit is mouth leaks so you'll want to test out a couple different methods to keep your mouth closed at night. Each time you have a leak like that you're likely causing arousals which are fragmenting your sleep. When you wake up in the morning is your mouth still taped? Do you use humidity with your bilevel? If so that can cause the adhesive on the tape to stop working. 

The name of the game, if you have great AHI already (anything below 5 is good, below 1 is excellent) is reducing sleep fragmentation and consolidating REM sleep every night which is what I'm working on and think you'll likely benefit from as well. I wouldn't focus on flow limits and trying to get your AHI to 0.00 because it's a strategy that does not typically bear fruit.

I do feel worse and had to have a morning recovery nap. Not awful, just not as good as previous day. 

What do you think I should do with my settings? Also, you say Ipap is 3, but It says 15? Im confused on that. 

Should I increase Epap to 8? And still increase PS to 4? 

That way the hypopneas may be taken care of? 

The leaks were a little baffling as I taped my mouth better than previous night, but the only change I can think of was the supine inclined sleep creating gravity on my mouth pulling open.
