Thanks so much, Sleeprider, for the settings suggestions. I've already changed the settings for tonight. I'll consider all the info on possible positional causes too.

In the OSCAR screenshots above, the pressure does seem to be fixed, as you said. But how can it be fixed if the screenshots also show that the device has been been in VAuto mode? Also, when I went to confirm your PS setting of 4.0, I believe it was set at 2.0, even though the OSCAR screenshots show a PS of 4.0.

*Can the higher pressure settings you suggested cause an increase in CSA events?

* Is there any info in the titration studies above that indicates that higher pressure triggered CSA events?

* Are my CSA events caused by the PAP therapy itself?

* No changes to TiMax and TiMin?

Your CSA events are probably just an artifact of low pressure and obstruction. We will know tomorrow. I'll look at TiMax tomorrow, you don't need any changes to TiMin.

Here are five OSCAR screenshots for last night, March 11, 2024.

I see 3 obstructive events, 7 CA events, 13 hypopneas, and 2 unclassified events, but there were unfortunately 65 large leaks, including one or two large leaks when I got up to go to the bathroom once or twice and left the device on. I know I have to resolve the problem with the large leaks.

* Did any of the large leaks lead to some apnea events?

* Did any large leaks lead to a failure to report some apnea events?

* Should I turn the device off when I get up to go to the bathroom?

I use ResMed Swift FX nasal pillows because I felt a suffocating feeling with a full mask.

* Would I not feel the suffocating feeling with a full mask now because you increased the pressure settings?

* What do you advise to resolve the problem with large leaks if I breathe through my mouth?

Additional settings:
 
TiMax: 2.0 seconds
TiMin: 0.30 seconds
Trigger: High
 
I'll be getting a full mask soon because I know I have too many large leaks.
 
* Can you please suggest further pressure adjustments?
 
* Is it possible that some events weren't flagged during the large leaks?
 
* Is my AirCurve 10 VAuto the appropriate device for me?
 
I'm asking because I've been approved for a new PAP device, and I want to know if I should get the same device I have now.

JonnyO, how old is your mask? You have a lot of leaks, and I wanted to start with that. I used to use the Swift FX and it was the best for a while, but I left it behind in 2015 for the quieter Airfit P10.

Thanks Sleeprider! I have the Swift FX nasal pillows mask, and it's only a month old.
 
As I mentioned before, I avoided a full mask because I felt a suffocating feeling with a full mask. But I didn't tolerate the full mask maybe because the EPAP was too low at 6.0 before you increased it to 8.0.
 
Is the P10 or any other nasal mask going to work for me if I am or may be a mouth breather?

The Airfit P10 is an evolution of the Swift FX design, and mainly differs in having a diffused exhaust vent that is very quiet and won't blow a stream of air on you or anyone in bed with you. The Airfit P30i is a newer mask in the same category that moves the diffused vent to a swivel on top of the mask. All of these are similar, and none of these can equalize pressure in the mouth if you don't control pressure in your airway during sleep. Some things that can help your conscious mind learn to control air is learn to speak normally while under mask pressure, drinking water with a straw, or being able to open and close your mouth without losing air. All of this teaches pressure control and many have this translate to sleep. If you jaw relaxes and causes leaks, a jaw-strap that pull upward, not back can be helpful, or even pulling the corner of a flexible pillow between your shoulder and jaw for support when laying on your side. If none of that works, a full-face with a hybrid design like the new Airfit F40 may work for you. It has a mouth cup with nasal pillows and leaves your face and nasal bridge free of mask parts.

I'm going to try the first things you suggested to control pressure in my airway and prevent large leaks during sleep. If that isn't effective, I'll ask my doctor and DME to get me an Airfit F40, which I didn't even know about. It looks good!
 
The doctor will order the mask and a new PAP device at the same time. Does it appear to you that my AirCurve 10 VAuto, which is more than five years old now, is still the appropriate device for me?
