RE: seems like I need to raise max pressure? - yankees123 - 11-19-2019

I am using the resmed aircurve 10 vauto.  My ahi numbers are almost always below 1.0.
My biggest problem is that the pressure when it starts to increase wakes me up during the night which I believe is from flow limitations.  This has been a recurrent problem since I began cpap  and I believe is related to my flow limitations.  I have limited my max pressure at the present level so  that I don't suffer from aerophagia and have the flow limitations run my pressure to the max.  What seems to happen is when I get flow limitations my pressure goes up as well as the mask begins to leak, which is what actually wakes me up.
To give a background I started with the resmed autoset but due to the fl. and only with epr max of 3, got the vauto to try and limit the fl. by increasing the pressure support.  I am currently PS of 4.8 and epap 9.4, and ipap 16.2.
I want to sleep thru the night and not have the fl. wake me up when the pressure starts to increase.
So my question is should I increase the PS to 5.0 to help with reducing the fl. or should something else be done? My OA are being taken care of with the epap I presently have,so I am not sure the best way to proceed.
Thanks very much for your time.
I have uploaded  3 screenshots and my statistic page, if there is anything else you need please let me know.

Increasing PS is the logical approach to reducing FL, and it looks like you could drop your EPAP to 9.0 without a lot of consequences. What is your trigger sensitivity?

Thanks for responding SR.  I have all the options on default (medium).  Can you tell me what you recommend and why?  Thanks.
Will reducing my epap help with the flow limitations that seem to wake me up?
Always appreciate your help.

Another question sleeprider is how can you tell that an epap of 9 would still take care of my OA?

This is just a observation/suggestion. I see you’re using a dreamware full face. I do not like that mask at all and it just happens to be because every time my pressure got relatively high I woke up. I came to realize it was because it’s simply harder to exhale with that mask at high pressures. If you really like that style mask giving the ResMed F30 a try just may solve your problem that you appear to be having for a very long time now. I have since moved on to nasal pillows but I’m def keeping my F20 and F30 around for anytime I have a cold and am unable to use nasal pillows. Something tells me it may be just that simple to fix your inability to stay asleep when pressures rise

Thanks for your response, I have tried the f30 any just could not get it to work.  I like the mask but just too many straps to get right and stop the leaks.

If you still have it adjust the top strap so the head rest right above your ears (the part that attaches to your head not the actual top of the ear). Then tighten the other 4 it it’s firm but comfortable. Turn on maskfit in your machines settings menu and tighten the top strap from the mask till it stops leaking. Then the bottom. If it’s still leaking from the nose tighten that strap just a smidge more. In your case it may be worth the struggle of getting it right cause my respiratory therapist was not surprised at all when I said it wakes me up when the pressures are high because he’s heard it before. The ResMed masks are night and day.

Let's just keep pressures the same for now, and try moving the trigger sensitivity to high.  I found that helped me, but it's going to be a personal comfort thing.  Since your therapy is very good, I'd like to try some small moves to see if we can address any discomfort without upsetting your balance between pressure and aerophagia.

Thanks Osiri I still have the mask and will give what you said a try.
Sleeprider, I agree to go slowly for now, will make the trigger change and report back results in a few days.  Thanks for your help.
