RE: Please help with treatment review - DaveL - 08-29-2020

Thanks. Last night I set up the autoset on the S10. It had EPR of 3

I was concerned that a previous post was misleading and not helpful. So I reposted with it arranged for direct comparison with he autoset post.

Your post was very helpful.

(I was challenged last night. I actually woke up twice. Second time I took my Knightsbridge band off, and had to reset the head gear on my N20.)

How does the use of EPR feel to you? You have been using fixed pressure without EPR for a long time, and if needed, we can more slowly transition into autoset or use fixed pressure as you did the first night.

RE: Please help with treatment review - DaveL - 08-29-2020

EPR was fine once I settled down to sleep...I'll keep it at 3 for now.

I'm thinking that I'll put the Knightsbridge on AND use my under chin cushion. I'll be careful setting up my head gear for the KB strap.

I'm not happy with AHI=2.5....

S10 Autoset for her SETUP - DaveL - 08-29-2020

(08-28-2020, 09:15 AM)Sleeprider Wrote: Let's try something new.  Autoset mode, at minimum 12, maximum 16 and EPR full time at 3.  If you have a known sensitivity to EPR use a lower setting, but use something.

Thanks Sleeprider

I've put those settings in. I've used Autoset For Her.  What's the difference?

Dave you are at a point where the numbers mean very little. So I ask you to ignore your number and go based on how you feel.

3.01 AHI - DaveL - 08-30-2020

Good morning!

First,,..I really like the S10.

Last night I put on my mask; lay down. Felt like I didn't have enough air.Same as night before.  I increased pressure.

I used the Knightsbridge strap. I put my under-chin cushion on.  Felt weird.

Here's the results. Don't know what to make of this.

edit:

Thanks Bonjour. I just read your post again.

Feelings:  I'm grateful that I'm on a path to imrpove my health...I'm grateful that I have a brand new machine; that it's a ResMed S10 autosens (IN SPITE OF THE ONTARIO SYSTEM)

All of those are important steps for me.  It's been a goal to get  an a pap for over 20 years.

I'm happy with the new Knightsbridge strap.

Feelings?  Yesterday my second mother passed away. She led a full life, and brought out the best in people.  RIP Isobel.  I used to call her Itsabel when I was an infant.  I'm 73...it's ok. She was ready to go.
Feelings?  I'll lick this auto pap stuff!

More info - DaveL - 08-30-2020

I've always had very low AHI's with my S9

Almost always below 0.5

It bugs me that I'm up to 3.0! using the S10 as an autoset...

RE: Please help with treatment review - Sleeprider - 08-30-2020

Dave, you were using the previous CPAP at a pressure of 13.0 without EPR, and your first comment on using the S10, is that pressure was too low as you went to sleep.  I think we need to increase minimum pressure to 15.0 with EPR 3. This is going give you the higher EPAP of 12.0 that you are accustomed to, and that prevents the OA.  Use a maximum pressure of 18.  I wanted a trial at lower pressure so we could see what happens. I do want to point out that your current AHI is not inconsistent with previous fixed pressure theray with no EPR. This session from August 13 had an AHI of 2.52, but note that your tidal volume is 360 mL with a minute vent of 6.63 L/min and respiratory rate of 17.2  Your most recent results with the S10 included an AHI of 3.0 but the tidal volume is remarkably higher at 420 mL and minute vent at 6.75 and respiratory rate of 16.  So we can see, the new settings have slowed and increased your breathing. As far as the higher AHI, I am seeing some positional influence here, and if you avoid the chin tucking, I think your AHI would drop, regardless of pressure.

Last night I incresaed min pressure to 14, and max to 17

I shall increase pressure right now for tonight to 15 with EPR3, and max 18.  I just sat through a morning long Zoom session.  Amazing:  I never fell asleep!. I believe that it's tidal volume and minute vent, and respiratory rate changes. Thank you!

Higher pressure: bad AHI - DaveL - 08-31-2020

Good morning,

Here are 2 screenshots showing last night's sleep.

Not compliant.  AHI too high.
