I don't necessarily trust the numbers and prefer to look at representative samples of the flow rate chart at a zoom level of 3-4 minutes. I have found that the flow limit index is a relative metric and shows how much flow limitation increases for an individual, and is not comparable to other individuals. We often shoot for a 95% flow limit below 0.10, but I have moved away from that without some analysis of the flow chart. If someone has persistently flat, flow limited inspiration they may have a very low flow limitation index, but someone with a normal, rounded inspiration curve with occasional flow limited periods will have a higher flow limit index. That said, in the mile-high view of flow, you seem to have a balanced inspiration / expiration and the 95% of 0.1 is very good as is the I:E ratio.

I wanted to update this thread on my recent changes and findings. After battling a slight cough and cold last night, I got a solid night of therapy in with the settings that @SleepRider has suggested. I switched to the F30i Mask from the F30, and it seems to have helped my eyes along with the eye drops that @SleepRider recommended. I also changed the humidity from 'Auto' to 'Manual' and set it to '5' and upped the tube temp by one degree as well, and no rainout and helped a little with dry mouth.
I think things are trending in a good direction after the tweaks in the past few weeks and about nine months into PAP therapy. Attached is an OSCAR screenshot of last night, and if there are any additional tweaks, I am always open to suggestions. Thank you all!

Quick question.
On the ResMed Aircurve, is VAUTO mode 15.0/5.0 PS 3.0 the same as on a Resmed APAP 5.0-15.0 EPR 3.0?

No. EPR is subtracted from the IPAP pressure for the EPAP pressure. PS is added to the EPAP pressure for the IPAP pressure.
Ergo, Vauto of EPAP 5 IPAP max 15 PS 3 is equal to Autoset range 8-15 EPR 3. For Vauto add PS of 3 to the EPAP (5+3=8 IPAP) up to IPAP max (15). For Autoset subtract 3 from IPAP pressure (8-3=5 for EPAP) up to IPAP pressure max of 15.
Confusing enough?

No. In bilevel terms, the Vauto is working at 8.0/5.0 to 15.0/12.0;
the Resmed APAP settings of 5-15 with EPR 3 are 5.0/4.0 to 15.0/12.0. So until pressure is over 8.0 you won't see the minimum Vauto pressure of 8.0/5.0. In other words, the low pressures on the Autoset are essentially unresponsive below 7.0 cm in terms of raising EPAP. Also the Vauto offers fine-tuning of TiMin, TiMax, and trigger/cycle sensitivity which is not available on the Autoset.

(I noticed SleepRider explained it before I posted, but I’ll do so anyway as it may fill in some of the finer details)
As CPAP pressure does not go below 4 cmH2O, if you have you Autoset (APAP) set to 5-15 EPR 3, the pressure would work as follows:
EPAP 4 IPAP 5
Then as pressure goes up for whatever reasons
EPAP 4 IPAP 6
EPAP 4 IPAP 7
EPAP 5 IPAP 8
EPAP 6 IPAP 9
And so on until IPAP max 15 is reached.
On the AirCurve the closest you can come is to set EPAP to 4 and IPAP max to 15 with a PS of 3. This results in:
EPAP 4 IPAP 7
EPAP 5 IPAP 8
EPAP 6 IPAP 9
Until IPAP max of 15 is reached. As you can see not quite the same thing.

Ok this all makes sense now. Thank you for the input. My point to this question if you were wondering is I am trying to adjust my Aircurve to the lowest possible pressures to get benefit. The reason behind this is complicated, but the theory is the higher pressures are an issue for me personally causing other problems. I was not having issues when I was on my cpap, but my AHI I thought could be improved, it was always below 5, but in the 1-3 range with many hypopneas.
Last night I adjusted my Aircurve to 15.0/4.0 PS4.0. If someone has an idea to get it closer to my old setting on the Dreamstation (15.0-5.0, CFLEX+1) OR to even further simplify my settings, please feel free to suggest, many of you have been of great help!
