I have some good news to tell you about my ResMed AirCurve 10 ASV device.

Last night, for the first time, I used a new ResMed AirTouch F20 full mask instead of my DreamWear under-the-nose nasal mask and my DreamWear under-the-nose (hybrid) full mask, and it made all the difference in the world!

• Flimsy under-the-nose masks are like cheap toys compared to a sturdy, effective full mask.

• I had a great mask seal.

• I had zero large leaks.

• Whereas I had usually struggled to sleep an hour or so while using the DreamWear under-the-nose (hybrid) full mask, I slept 7 hours and 21 minutes last night while using my new AirTouch F20 full mask.

• I had minimal events and significantly improved oxygen saturation levels.

• I woke up feeling much more refreshed.

• Although I had to go to the bathroom two or three times, I was able to fall asleep again without the device giving me overwhelming surges in air pressure.

• When the device increased the pressure a little too much for me, the blow-back strategy was surprisingly effective.

• When I woke up this morning and was breathing naturally, I wasn't even sure the ASV device was on and delivering any air pressure and had to hold my breath for a couple of seconds to see if the device was on and would increase the pressure.

I noticed that both of you use nasal masks. Have you tried a full mask with your ASV device?

I think using a full mask makes much more sense for many people than using a flimsy nasal mask that isn't as effective, is prone to leaks, and requires taping your mouth closed to prevent mouth leaks.

I hope that my success with my AirCurve 10 ASV last night will continue tonight and thereafter and that some device setting adjustments will improve my sleep even more. I also hope to post some more OSCAR screenshots soon.

Devin, I am thinking of try the F20, do you side sleep or sleep on your back?

Hi stanleyjack, I prefer to sleep on my side but was sleeping on my back because I was afraid I'd get large leaks when sleeping on my side.

But last night, I tried sleeping on my side and had no large leaks with my AirTouch F20. Getting a special PAP pillow will help too.

I had the straps a little too tight and had some periorbital edema as a result. So, I'll make it less tight tonight.

This is a super informative thread! I finally got my ASV this week, so this is a great read. I've definitely noticed more air blast wake ups with the ASV. Its max pressure is way higher than the 5-7 range I had on my APAP. I had one night where it felt like my hybrid F30i spent a few hours hovering just above my face, and I kept pushing it down with my hand until I relaxed in my sleep (the OSCAR charts confirmed the leaks). The next night I woke up enough to rewipe the cushion, and that seemed to help it stick for the rest of the night. There have also been hours where I slept just fine and had no leaks. Using a mask liner doesn't seem to make much difference, which surprised me.

Based on your experience, Devin, I'll try the F20 mask tonight and see if that changes anything.

Hi Dani, thanks for your comments.

I've been trying out lower pressures settings with my AirCurve 10 ASV and have found that the intolerable surges in pressure are much better and that I can now sleep for seven to eight hours. Once I'm asleep, the lower pressure settings still result in very few apnea events.

My real problem, though, is that I experience a series of central apnea events that occur mainly while falling asleep, which I've learned are not uncommon:

Although the AirTouch F20 mask is effective for me, the replaceable foam cushion is not so durable. I just ordered an AirFit F20 silicon cushion to try with the F20 mask system to see if it is effective enough and more durable.

Good info!

I tried to make the F20 (silicone) work for me - I just couldn't make it comfortable and non-leaky last night.  I don't remember it being painful on my nose during my overnight sleep study.  Maybe it's lost its flexibility over 6 months.  I tried using the ramp feature (15 min) last night to deal with the blasts and that was a BAD idea.  Looks like I've also got the falling asleep apnea like you describe!  Which is weird, because on the APAP, my apneas clustered toward the end.  I'll probably try decreasing the uppermost pressure after I give it a week to adjust.  

How do you know your hypnogagic apneas are central?  I didn't think the ASV would record central apneas.  Mine definitely *feel* central, but I'm not sure how to actually determine that.

Using the same mask cushion for six months is a long time. I'll try the silicon cushion and see if it works for me. At least, it works with my F2 mask system. 

I know I have central apnea while falling sleep because I catch myself not breathing at all and not making any effort to breathe for a period of time. Also, you can look at the flow-rate waveforms to see what type of UA events they are. 

I see you have wide pressure ranges. Many here recommend narrower pressure ranges because the wide ranges can lead to various problems and the device may not auto-titrate as it was designed to do. 

You can post your OSCAR screenshots for experts to analyze for you.

I've been using the hybrid F30i and replacing cushions regularly, I just happened to have the F20 they gave me with my sleep study in December.  So perhaps it doesn't fit the same because it's lost some elasticity?  Or possibly I'm cranking down on the mask straps because of the higher pressure.  

I'll have to update my therapy thread when I get some good data.  Big Grin
