You are correct that it is a balancing act. As indicated in the sleep study I summarized in the post. The main issue is the high RDI and the high AHI exclusively during rem sleep. My overall ahi was like 6.5 or smt like that. So I think only looking at ahi is not relevant in my case. Also, I think the threshold of 5 is just an arbitrary number. It's kind of hard to manage this aerophagia cuz I have bad burps, but I don't recall them waking me up from sleep, but it might be disrupting sleep. Thank you for the feedback, I just made the post to make sure there wasn't something I haven't thought about or considered in this complex issue. Working on getting that bipap somehow to maximize health. RE: Oscar Data. No one can answer that. But if you want to get a bilevel machine anyhow, without any guarantee that it will help you, I think the ResMed Aircurve 10 VAuto is an excellent machine. RE: Oscar Data. No one can answer that. The reason I think it will help is the following: During my test, i got an RDI of 23 and got diagnosed with UARS and mild sleep apnea (moderate during rem or supine position). My flow limits spike drastically during each rem cycle, and reduced greatly with the epr setting. However, they are still present. I know that I am not breathing very well because I feel very tired when I wake up and throughout the day (could be because of other factors that disrupt sleep - no way to be sure) and I often wake up for a few seconds during the night or early morning with a very dry mouth. Also, not to mention the flow limiations and the absolutely erratic and non regular flow rate with no consistency lots of flattening... during rem sleep. If epr reduced flow limitations during rem sleep which is correlated to my diagnosis of Severe UARS exclusively during rem sleep, then wouldn't more pressure support further reduce the remaining flow limitations?
