RE: Please help with treatment review - grizwilson - 01-21-2022 Need recommendations, Attached are screenshots from 1/17 dreamwear no collar, 1/19 Mirage with collar and 1/20 dreamwear with collar. My leaks go down with collar, guessing either holding chin up or enough lift to keep from moving mask during night. But AHI goes up with collar, as Dave has said likely due to more accurate data with fewer leaks. My question is should I continue with the dreamwear and collar or should I keep tweaking? However, I've mess ed up my treatment trying different masks. P30i and F30 or whatever the full face mask is. So I did a thorough cleaning with my N20 mask insert. Zeiss wipes. And I taped last night. Solved my mask and lip leaks. I used my old home made pad under my chin. It's more comfortable than my soft cervical collar I bought from amazon canada. How do I feel? Went within and cleared a whole lot of emotion. Applying engineering principles. 1. If it ain't broke don't fix it. 2. If it doesn't fit get a bigger hammer. Settings on the S10 are about the same as when I got good results. I know for a fact if I take my results to a (my) sleep doc he's going to say "what the heck? Your prescription is for a cpap. Not an apap. And your prescription is for 13. I feel caught between a rock and a hard place. RE: Please help with treatment review - SarcasticDave94 - 01-22-2022 The CA do present themselves nearly each night at varying amounts. Given that and you're not finding or gaining the well rested state, ASV has a lot of merit. I'm not sure if it's applicable where you are, but maybe present your history of treatment to the doc with the summary info. More important may be your laundry list of all the things you've tried, meeting failure to gain rest. How you feel it's supposed to be counted in the mix, and you've not gotten that for the many years you've invested. Request a change. RE: Please help with treatment review - Geer1 - 01-22-2022 CA levels are too low to be able to convince doctor there is an issue. I see you recently lowered pressure at the beginning of/during this issue. You should change it back to the same settings, mask etc used beginning of January. Goal is to try and stabilize breathing back to that same point and the easiest way to do that is to mimic what you were doing at that time. Although a number of these get flagged as central a number get flagged as obstructive and the lower pressure now being used may be an issue. RE: Please help with treatment review - sheepless - 01-22-2022 besides higher pressure potentially contributing to higher ca, the suggestion to reduce pressure was largely intended to help get some serious leaks under control. this 'catch 22' (ca vs oa, pressure vs leaks) is the reason for the suggestion to try a different modality since you've pretty much run the gamut with apap. no harm in trying previous settings again though. He thought 11 was enough back then, but that I would have more deep sleep. I haven't seen him in about 2 years. He's keen on sleep tests cuz he makes more money. I want to get my treatment under control. As it was before. |
