RE: Update (BiLevel) - Dread - 11-06-2023 Definitely. I noticed I've gained about 4 pounds. I've been trying to lose it and probably see things improve. When I gain weight, I notice my nasal blockage from the septum gets worse. Monday my endocrinologist wants to start me on TRT due to the fact my T is barely improving with treatment as well as androgel. RE: Update (BiLevel) - Jay51 - 11-07-2023 Dread, just one thought here. Possibly, when you change sleeping positions at night, it triggers POTS (which triggers the higher RR). It would be almost impossible to prove this because you are asleep. If it woke you up, you could record the exact time and check that against your OSCAR charts. Do you change positions during sleep? If you could somehow stay in only one position during the night and then check OSCAR charts, that may either confirm or deny it. When I had a Holter (heart) monitor one time, I went into tachycardia when I stood up. Not sure that happens with back to side sleeping movement though, etc. but a possibility. RE: Update (BiLevel) - Dread - 11-07-2023 Hey, Jay. I think it may be a case of movement? I just got a new wedge pillow last night, because my old was way too steep. Last night's RR showed 95%, 16.0 and 24.40 at 99.5% RE: Update (BiLevel) - Dread - 11-10-2023 Question. Im still having leaks / flow limits. Do I increase PS or EPAP? RE: Update (BiLevel) - Sleeprider - 11-10-2023 Neither leaks or flow limits will be improved by changing PS or EPAP. Leaiks are all about mask fit, hygiene and a good fit. We do not worry about flow limits in ASV that appear as individual spikes corresponding with breaths being assisted by PS. In the absence of a new graph, I'll assume they look like your last graph. RE: Update (BiLevel) - gainerfull - 11-10-2023 One thing that I found helpful in my bilevel therapy was increasing TiMax (I realize this is ASV titration but still). Based upon the full night charts your inspiration time is as long as 4 seconds! I'd probably increase TiMax from 2.0 to 3.0 as a trial as it would definitely better accommodate your median inspiration time. Now, unfortunately dread you have a bunch of biological factors working against you (lungs) and being realistic you may not be able to explain your rapid breathing episodes or consistently get a sub 1 AHI but your therapy has already gotten a lot better within this thread. Good luck RE: Update (BiLevel) - Sleeprider - 11-10-2023 No TiMax setting on the ASV. Long ago in this thread I argued that the Vauto would have been a more appropriate device. When central apnea is not an issue, as in this case, the ASV is just a kludge. RE: Update (BiLevel) - Dread - 11-18-2023 How do you effective tiltrate it. I feel like settings for mine are still suboptimal. RE: Update (BiLevel) - Dread - 11-18-2023 I’ll post Oscar data tomorrow RE: Update (BiLevel) - Dread - 11-18-2023 Like MIN PS MAX PS Min EPAP Max EPAP. EPAP for hyponeas, but when to increase PS, and how. Like when to raise min ps vs max and same with epap |
