Newbie needs help - Linda G - 08-23-2022

[attachment=44094][attachment=44093]Have been using CPAP for 2 yrs with large mask leaks (Amara view FF) and no physical improvement.  I am new to looking at data, using OSCAR and Bleep nasal device (which I really like), cervical collar and mouth tape (thank you forum members).  I have been getting better numbers but still feel exhausted all day.  Is there anything in my charts that might be a clue.  I am 78F, 150 lbs in good shape other than this issue.  Believe I have had OSA for 15 years but have not been successful with therapy yet.  Any advice is very welcome.  Thank you for having this Forum to discuss with.... have read a lot of threads and have learned a lot but just the tip of the iceberg likely.

You are using a bilevel machine with a pressure support of 5.0 cm and most of your events are centrals. I think if we gradually titrate your pressure support a bit lower, you should have better results. If you know how to enter your settings, try reducing pressure support (PS) from 5.0 to 4.4. This should reduce the tendency for central apnea and any periodic breathing. Overall, things look pretty good (without a good closeup analysis), but the PS seems high and is really cutting your inspiration time and has reduced flow limit to zero. A bit less might be better.

Thank you so very much for your advice.  I waited until tonight to reply because I normally have a chronic fatigue all day with a big dip in mid-late afternoon.  Today, 8/24, after making the adjustment you recommended is the very best I have felt all day in many years.  I was so close with the settings but not close enough.... you found the right combination.  So my plan is to do exactly the same thing tonight to see if I can get two days in a row - miracle.  If so, would you recommend lowering PS a little more, say to 4.2, or do you think it is ok at 4.4?  Data below from 8/23 with your change.  I was awake failing to fall asleep for two hours in the beginning and I think you can see that.  Afterwards, normal sleep, a little restless but slept through.  Usually tired in the morning and can sleep for another 2 hrs easily.  That will probably go away with this improvement and better sleep.  Thank you again--I am so happy today.
