Clear Airway (CA) Question - Reesche - 08-20-2023 Hi you guys.... I have been using my ResMed BiPAP for 4 months now. I am still waking up feeling tired and out of breath. Yet I am getting good numbers.... See attached Screen shots of last night. DATA: I look at ResMed's MyAir and my numbers look good. I just went through a Mask Change.. From ResMed F30i to Fisher & Paykel "Evora" and it eliminated a bunch of the mask leaks. However, viewing my data in Oscar, it shows less obstructive Apneas but the Clear Air Way (CA) Apnea where the airway is open is still pretty high. Talked to the Respiratory Therapist the other day and she said my "Central Apneas" are still high. Is the graph in Oscar... Clear Air Way (CA) Apnea where the airway is open the same as Central Apneas in Oscar? They use a different application on their end. Thank you, looking forward to your reply. See Attachments.... Reesche RE: Clear Airway (CA) Question - Gideon - 08-20-2023 Looks to me like a form of positional apnea. The key is the clusters of events. Likely this is you tucking your chin. Try a single pillow or a flatter, less firm pillow to keep your head and neck straighter, thee are more aggressive solutions if needed. The clusters of CA events could be the same thing, so post a 2-3 minute view so we can see what is going on. Please use the standard order for your charts. Note that the AHI chart is basicly worthless for us to see what is going on, we see it in the events chart View/reset graphs/ standard from the menu take the screenshot with F12 windos, or FN+F12 on a Mac as that does some temp formating. Clear airway events are shown as hash marks on the Events Flags graphic. There is no similar mark for Central events. That said, central apnea is a diagnosis, while Clear Airway is a clinical sign (in much the same manner as elevated pulse rate is a sign, but it does not necessarily mean that the diagnosis is an infection.) CPAP-APAP therapy can do little to treat an actual central apnea condition. (Actual central apnea is best accommodated by a ventilator - but that is not a treatment) So, if your medical professional is serious that you are afflicted with central apnea, you should press for a exam that will identify and address the underlying cause. RE: Clear Airway (CA) Question - Gideon - 08-20-2023 In general you can assume that CA events are central, but verify first. There are often tells in the Flow rate chart that can be used to prove that they are infact not central. Without question they are not obstructive events, but often are simply the pap user holding their breath while tossing. This holds true for any form of pap machine. The ASV, the only pap machine that is specifically designed to treat mixed/complex/ idieopathic central apnea doesn't even have a mechanism to detect central apnea.
