Help with bipap - Al N - 09-14-2023
Dr is semi retired and seldom in his office I have been on cpap for over ten years. Used it every day. Used with nasal pillow mask for several years. Number were good AHI almost always below 5. Have always struggled with being tired even with cpap
In a visit with my neurologist a few months ago. He suggested a new sleep study. It was supposed to be overnight and then the next day. As it turned out they spent the first few hours trying to get me to sleep without any cpap at all. Didn’t work couldn’t sleep. Finally they brought in a cpap machine and hooked my up. I still had trouble getting to sleep. Finally slept some. At about 4:30 was back awake. They came in unhooked everything and sent me home and said I didn’t get enough sleep to qualify for the next step. Asked me if I felt more refreshed than usual. I replied no not at all.
What I didn’t realize is that they had switched me over to a bibap machine over night with pressure of 24/20. I had previously been on a pressure of 14. Fast forward over several months of waiting
Dr prescribes a bipap with the suggested setting. While waiting on the insurance approval I did raise my cpap pressure up In auto mode to between 14 and 18. I actually felt a little more rested
Finally got the new machine switched from nasal mask to a hybrid full because I thought maybe I had been getting inaccurate readings on my old machine since they always showed consistently controlled apnea. So new Aircurve 10 vpap machine. New mask. Horrible results. I seem to sleep good. My readings on machine at best are ahi of 17 and sometimes as high as 45
Saw the dr again a week ago he advised dropping the high end pressure down to 18. Readings are worse. Mask fit seems good. And most all episodes detected are apneas not central’s or hypopneas.
Trying to understand why the results are worse. Now whether it was bad results before or now

It would really help if you will download the free OSCAR program and submit some charts of your therapy. On second thought the S9 Elite does not provide detailed efficacy data. Okay, while you’re adjusting settings, please be sure to turn on EPR full time at setting 3. That gives your machine limited bilevel capabilities of 3-cm pressure support (EPR). If you’re not already using it, it should help a lot. About all you can report on is AHI and comfort, but would be good to know.

I will work on getting that downloaded. Sleep quality doesn’t seem to be any worse.

No, OSCAR requires a computer. An iMac, Windows, Linux, or Chromebook are your current choices.

MyAir is simply useless. If you can find a way to borrow or obtain a computer that can operate Oscar, it will be more insightful. Is there an SD card in the machine to record data?

No sd card. I did switch s mode. For now at least. iPap of 25 and epap of 21. With a very small sample size so far seems to have helped. On the vauto they had the settings at 18 and 4 with the ps at 4
