Can't keep mask on all night - emash124 - 09-15-2023

Hello everyone, I have been using an Aircurve 10 ASV for the past two months and have not been able to keep my mask on or myself asleep for more than 2-3 hours at a time. I use either a Brevida, N20 or N30i, I prefer the N30i as I am a side sleeper but I rotate them around. I use a chin strap as well, and have the Knightsbridge on the way. When I wake up, I am not in any distress so it is confusing why this keeps happening. I have been using CPAP/BiPAP/ASV now for over a year, and the ASV has been the most difficult to adjust to. I also have a hose holder above my bed to help with tangling. I could really use some suggestions, thanks for your help in advance. Just FYI, I have mild to moderate CSA which is why I use an ASV. My sleep study only showed 0.1 OSA events. I have never tried ASV mode, just ASVAuto. any help would be greatly appreciated!!

PS-I know an index of 0.00 is considered awesome, but I don't see the point if I can only keep my machine on for 2-3 hours every night. Any increased comfort measures or things y'all can point out that might be affecting my compliance would be greatly appreciated!

Welcome to ApneaBoard.  I have used ASV before.  The only thing that stands out to me in all 3 of your charts is that your tidal volume is low in your 3rd chart.  Looking at your flow chart, when problems arise, the ASV responds with more pressure.  That is the way this particular machine is supposed to react.  Your EPAP stays really low all night.  Your pressure support kicks in to stop CA's.  This looks to me like this is just exactly the way this machine is supposed to react.  

If someone else sees anything, please post.  You might actually try lowering your PS max (max IPAP) as low as you can and still have good charts (not many events).  Maybe lowering the pressure might help you stay asleep.

Just my opinion, you'll probably want to stay on ASV Auto mode. It was far more comfortable than straight ASV for me. Others far better than I will chip in, but I'm wondering if you need to keep EPAP MIN at 4, you might need more air as your base than you think. If so you'll want to increase that min a click or two.

here was a couple nights ago... kept the mask on for a bit longer but my sleep was terrible for some reason (woke up groggy as heck, apple watch reported low sleep quality, etc...) any ideas on why I keep waking up? It's getting almost intolerable at this point.

Just my opinion, you may want to add 1 to the minimum PS, unless you've tried and it made things worse. I did better with 3 PS min when on ASV, although I'd tried min PS 0 and didn't like the feel of less air on inhale.
