Hi There,



I am hopeful tha somebody is able to make some sense of the data i have extracted over two days whilst wearing a full face mask using a S9 Escape machine.  There seems to be similar event over the 2 days happening around the same time and am not even sure if i have sleep issues or not (no official diagnosis).  I would be grateful if anyone more experienced has some insight , it would be much appreciated.

your overall data is not so bad, but there are a few things we could improve. First, your leak rate is way too high, to a point where it is likely compromising the pressure delivered by the machine and, perhaps more importantly, potentially also disturbing your sleep.

With regards to the clustered hypopneas we can see, that raises the flag for positional apnea. In other words, you're likely moving into a position where your throat is obstructed by your own body beyond what the machine's pressure can resolve, perhaps rolling onto your back or crowding your neck somehow. You could try a soft-cervical collar to narrow in on the neck possibility. Do you sleep on your back at all?

Hey CpapFriend thanks for the response.  I actually almost always sleep on my side tbh and on my left side as well , i don't know why but i just do i suppose.  I appreciate what you are saying about leaks and will try and address that although the mask is pretty tight already on my face so perhaps lower the pressure ? I wonder if i would even have a diagnosis or is it likely i would probably get a diagnosis if i saw a consultant i wonder.

To be honest, currently you're using the machine as a leaf blower, hence you can't rely on the other data provided by the machine.

Remember: a tight mask is *always* wrong.

While you are sleeping, do you feel that you get awakened by the noise or the rushing breeze?  Are you aware of adjusting the mask to resolve the leaking? and, do you know where the leaking is occuring? (into you eyes, mid-nose side leakage, mouth or chin leakage?)

Suggestions: wash the seal or replace the cushion, wear a cotton mask liner, learn to adjust your mask properly. others may depend on whether you know where the leak is originating from.

What type of full face mask are you using?  I just switched to the AirTouch F20, and my provider told me to make sure the bottom of the mask is in the crease of the chin.  This means the top of the mask will be almost between your eyebrows.  It felt weird the first couple of times, but it provided a perfect seal.  Also, while holding the mask in the proper position with one hand, adjust the straps with your other hand so they're just tight enough to get rid of the slack.  If you're saying it's already tight, making it even tighter will probably make the seal worse not better, since the mask won't be able to inflate properly.

See how flat your leak chart is? That indicates a very even leakage. Typically from mouth breathing. You are using a FFM which means it is likely a poor fit. read the Mask Primer, especially the sections on mask fitting. Modern masks all have a flap of silicon that actually make the seal when your cpap is on from the pressure that is produced. ALL the straps should be doing is holding the mask in place. Your reference to "Tight" indicate you are doing this wrong. A "tight" mask tends to change the facial geometry and actually causes meaks. This is a major cause of leaks.

The FL portion of your chart say you would benefit from EPR=3, Full time, This also requires you to increase your min pressure setting to take full benefit and results in an actual min pressure of 4. This should be more comfortable for you.

Should he also raise his high-pressure setting? On the first chart, he seems to be repeatedly bumping up against it.

Your pressure is raising to the max because of flow limits.  Gideon said to put EPR to 3, on your machine that is as high it will go.
