Need Help analyzing OSCAR data - megagreatwhite - 10-01-2023
Would someone be able to help review my OSCAR data from last night? I started my CPAP therapy little over a week ago. I switched to try nasal pillows mask Resmed P30I and machine tells me great mask seal and my leaks have went down from when I was first trying a full face mask. My AHI in my sleep study was 70 and last night got AHI 18 with most saying clear airway. I can share other days data as well if it would help.

In your sleep study, were most centrals there also?" If you could block out all personal information and post the sleep study. The machine you have does not control central apnea. However a cpap machine is not a good judge of centrals. In the sleep study they hooked you up with wires attached all over your body and put a strap on your chest to see what type of apnea you have so your test is a good predictor of what type you are having now.

Yes, sleep study found evidence of severe central sleep apnea with a significant of the events being of obstructive origin. I am attaching sleep study results with personal info removed. When I brought the pdf into word to remove personal info text formatting got messed up slightly but should still be readable on the screenshots.

Your test results would suggest that you are an ASV candidate. Using the EPR on your current machine is adding to your CAs, by washing out your CO2. Have you tried setting it to 0 (turning it off)?

No, not since I switched to the nasal pillow mask. I will try that tonight. I also switched from auto 4-20 to cpap of 12. Should I stay at static pressure level of 12 or switch back to auto (what range do you recommend if so)? I also have ordered some mouth tape that I was going to try in case I have any mouth leaks.

I wouldn't change too many things at once. Since you have data for the static 12cm with EPR, comparing it to non-EPR would be a fairer comparison.

Sound's like you need to ask your doctor for an ASV, or try to get a sleep study that shows at least 50% centrals (hint: insist sleeping on your sides if you normally do so) ASV often works most extra for you, if you have more central apneas, especially sleeping mostly on your side or stomach. ASV is pretty much as is good as it gets for those with mostly central apnea, unless you have COPD, then you might need to ask your doctor about an iVAPS. As always, consult with your doctor.
It seems we often forget that at least 10-15 % of sleep apnea is central, and may be mixed with obeructive sleep apnea events.
