Nose irritation with nasal pillows (p10) - Jobaaayyy - 10-19-2023

I recently started using a cpap and got a p10 mask. I consider my face and nose size to be average, so I started with a medium attachment. The strap is fairly loose and it's otherwise comfortable. 

I was fine for the first couple of days, but then I started getting badly irritated inside of my nasal passages by the center cartilage line. I tried Vaseline, but I think I was too late. I have open, red sores inside of my nose and it's painful. It's discouraging because I have been making good progress in adjusting to the machine otherwise. 

I guess at this point all I can do is wait a few days to heal and then try again, this time with Vaseline from the start. 


Should I take this as a sign that it's just not going to work for me and give a cradle mask like an n30 a try instead?

You should use a different style mask until you heal.  

In the meantime, use some antibiotic ointment on the sores to promote healing.

It's possible you are using a nasal pillow that is too small.  The pillow shouldn't be inserted too far into your nostrils, just a tiny bit.  Try a size large pillow.

Also, once healed, invest in some nasal gel.  Place a tiny bit in your nostrils before masking up.  A use a brand called Ayr Nasal Gel.

Thanks. I never considered that the pillows might be too small. I can try the large size when I heal in a few days.

Would the n30 be a good alternative to try when things like this happen?

Yes, as long as it's not rubbing on or irritating the sores.

Note - vaseline is pretty horrible on any mucus membrane, it tends to dry things out and make them worse. Nasal moisturizer is the only way to go.

It's also important to be aware that Vaseline will degrade the silicone of the nasal pillows.

The biggest problem with the P10 is most people select a cushion size to small. I wish they had an Xlarge cushion size. I'd be their first customer.

I find that the biggest issue with the P10 for me is the flimsy head straps that refuse to stay in place unless I make some sort of ponytail in my hair. I had the chance to try an N20 mask last night and, wow, what a difference. No headgear/mask related arousals at all. I think if I had had a helpful sleep doctor in 2018 rather than one who told me to just ask my PCP for sleeping pills, issues like mask related arousals could have been resolved and I probably wouldn't have given up on CPAP therapy.

I started with the N20 and it was causing scabs on the edges of my nose.... I've since switched around a bunch of different masks and have landed back on the N20, but I switched to the airtouch (memory foam touches your face). Since then I've been below 1 on the AHI scale. Note - I was on a medium with the airfit but had to go large on the airtouch.

I do have to use a chinstrap with my N20 or I exhale through the mouth frequently.

When you can use the P10 again, instead of vaseline put some Lanishoh cream in your nostrils.  Lots of us have used this successfully.  You will find it in any pharmacy's baby department, WalMart, etc.  It is made for nursing mothers to put on nipples that are sore.  It is perfectly safe (babies suck on it) and it won't damage your mask.
