Need Help With Settings - UARS - Charts Included - Samthesleeper - 10-20-2023 Still feeling like crap. I have UARS, and need help adjusting my settings. Had a nasal mask but will be trying nasal pillows. Question: if my problem is to do with my tongue/epiglottis/palate is there a trick to the settings that I should do? Anyway, see the attached charts - Night 1 and Night 2 - from a few months back (coming back to Pap therapy after a break) I can ZOOM in on any portion to help out, just let me know. And Night 2 RE: Need Help With Settings - UARS - Charts Included - Samthesleeper - 10-20-2023 Also, are the attached examples of expiratory pressures? Like palette blocking on exhale? RE: Need Help With Settings - UARS - Charts Included - gainerfull - 10-21-2023 1. Your leak rate is sub-optimal - what are you doing to prevent your mouth from opening / leaking air at night? 2. How did you come to these settings? It looks like your EPAP (exhaust pressure) is 1cmH20 or so too low as evidenced by the obstructive events (light blue). 3. What is your trigger level set to? Looks like medium so you may consider changing it to "high" to help with the residual central apnea events. RE: Need Help With Settings - UARS - Charts Included - Samthesleeper - 10-21-2023 So should I raise Epap? Should my PS be the same or lowered? RE: Need Help With Settings - UARS - Charts Included - gainerfull - 10-21-2023 (10-21-2023, 10:49 AM)Samthesleeper Wrote: So should I raise Epap? Yes, I'd raise it by 1cmH20 and change trigger to high to see what it looks like and keep everything else the same so we manipulate the minimal amount of variables. Keep in mind these pressure changes are going to make leaks worse so you'll have to figure out what you want to do about your mouth opening at night. RE: Need Help With Settings - UARS - Charts Included - Samthesleeper - 10-23-2023 Okay, see the attached Oscar from last night, raised EPAP to 7, reduced PS to 4. I taped my mouth a little bit, and wore a collar for first half of night, think I took it off around 2:30ish/3ish but can't remember, I feel "OK". But I am still waking up more than I'd like, Any insights here? I will attach a close up of the flow before the centrals - are these 'true' centrals or a change of position? What should I do to decrease Flow Limitations? If these aren't 'true' centrals, does that mean I have more leeway on pressure? Thoughts much appreciated! RE: Need Help With Settings - UARS - Charts Included - gainerfull - 10-23-2023 Leaks look good - nice work and after the pressure bump we're only look at centrals which is great progress as well. Those spikes in the macro view of the flow graph look like RERAs which can be disruptive. What was your trigger setting set to last night? RE: Need Help With Settings - UARS - Charts Included - Samthesleeper - 10-23-2023 I set trigger to high from medium. Yeah I have a feeling the RERAs are what causes the spikes before the Centrals. Where should I go from here? I am going to wear the collar all night tonight. And also tape my mouth even more. Will also be sleeping on an incline pillow and see if that helps. Are my arousals inspiration related or expiration related? Should I show you any other graph areas? Thanks so much for the replies! RE: Need Help With Settings - UARS - Charts Included - Samthesleeper - 10-23-2023 Bumping this one. Anyone have any ideas on the above? RE: Need Help With Settings - UARS - Charts Included - Crimson Nape - 10-23-2023 First, upgrade to OSCAR, version 1.5.1. It provides additional CPAP information. If you need to show a sleep session, instead of zooming to the time frame, try scrolling down on the left side to Sleep Sessions and deselect the one(s) you don't want to include. In version 1.5.1, you will need to first go to OSCAR > Preferences > Clinical and select the Passive mode, before you can deselect a session. - Red |
