My Oscar Data - some guidance - Lotharen - 12-02-2023

Greetings everyone! I'm new to the forums and using Oscar. Been feeling a bit tired lately and trying to troubleshoot if its a CPAP problem. Hope someone can point me in the right direction or let me know if all looks good. I might be an unusual case in that I was started on CPAP therapy for Polycythemia and my last HGB/HCT came back a little high so I want to make sure the therapy is working. 

My Settings are as follows -
Pressure: 7-16
EPR: Ramp Only
EPR Level: 3
Ramp Pressure: 7
Response: Standard

Hi Lotharen!  -  Welcome
My initial recommendation would be to:
1. Turn off the Ramp
2. Set EPR to 3 and Full Time
3. Set your pressure range to 11 - 13

Using the ramp is delaying your therapy.  Your flow limitations are pushing up your pressure.  The EPR of 3 full time should help in reducing the limitations.  I listed only a 2cm pressure bandwidth to avoid pressure change arousals.

Try this for a night and see if you and your results improve.  We can always tweak these settings.

I don't feel to bad. Little groggy this morning when I first woke up but not passing out tired between 4-5 like I have been most days.

I noticed that the one difference I saw was that the Pressure section had 2 lines. Before it was only one. I assume the settings helped with that? Just curious as I want to understand it a bit more.

Also, I see that a few times I capped out at the 13 pressure, does that mean we need to adjust that? or is it fine?

The red line is your inhale pressure and the green line is your exhale pressure. If your events were common throughout the night, then a pressure increase would be in order. Yours would seem to indicate positional events. These can be caused by turning, chin tucking, or sleeping on your back.

Okay, its been a bit since my last post. Generally sleep okay. My wife did say she heard me snoring when she came to bed a few nights ago. So I wanted to post on more oscar capture from last night. Could that have been a fluke? or should something be refined?

I'm wondering if I need to increase the max pressure back to 16 with results of the previous file I attached. I believe disabling the ramp pressure was the best thing so far but I'm not understanding the narrow low/high pressure setting that was recommended. Specially since my wife heard me snoring the other night.

Your flow limit at 95% might make your CPAP go to your highest setting. To prevent a large pressure swing that may disrupt your sleep, you might try a pressure range of 12 to 14. Keep everything else the same.
