New CPAP user - OSCAR Data - Bourgi - 01-09-2024

Hello, 

I did a sleep study due to my partner unable to sleep next to me and letting me know I was snoring and gasping for air. I had no idea Sad

I did my sleep study through LOFTA and got diagnosed at 52.6 API. I ordered the ResMed AirSense 10 and used it the first night as is, with I think 4-20. This hurt my chest too much and I was struggling, so I set it to 4-15 and felt much better but still fidgeting with the mask and waking up intermittently. I skipped the 3rd night because I was getting blisters on my nose bridge. 

4th night I finally got an SD card. I was able to sleep through most of the night. Attached is my data.

You are doing well using those settings. Let’s fine-tune a bit.

Min pressure 7
Max pressure 15
EPR 3 full-time
No ramp

That should help with the flow limitations, stop most of the pressure swings, and hopefully be much more comfortable. Give it a try for a few nights and let us know how it goes.

Your CA events are lower than mine and I've been told that I have no reason to be overly concerned. I'm not giving you advice, merely sharing what I've been told.

From your graphs it looks like most of your CAs are after a sustained pressure drop - so by increasing your Minimum pressure to 7 as suggested by PeaceLoveAndPizza might make some of the few, go away.

Trying out the settings as suggested by PeaceLoveandPizza, overall I think this setting works well in terms of comfort. Not sure about the OSCAR data however. It seems I'm still getting some flow limit issues?

Looking better.

Your median pressure is close to 11, so try the following:

Min pressure 10
Max pressure 16
EPR 3 full-time

A bit of a conundrum though. We can keeping tweaking and get things as comfortable as possible with your current machine. We will not be able to manage the flow limitations completely, but should hopefully be able to find something you can use to get a decent nights sleep. The one caveat is if your flow limitations continue as they currently are the machine will always want to keep driving up the pressure.

You have enough flow limitations to justify using a bilevel as you need more pressure support than your Airsense 10 can provide. You do not need it for the AHI but for comfort. If interested we can help with getting information put together to show the doctor. Take a look at this thread.

Thank you again. I should have kept with my low-deductible health insurance plan instead of switching over to the high-deductible ?.

Either way I'll keep trying out sleep therapy methods to make sleep better for me. I'll keep collecting data and making a case for insurance to cover the bipap if needed.

Hello again with an update after trying a couple weeks with Min 10 Max 16. 

Looks like I'm still getting flow issues Sad

I'm also having leak issues sometimes even with tightening of the mask. Wonder if my face is even too small for a small mask. Tried the F30 and that could not hold a seal at all, my nose is potentially to short? 

Not sure if I can use nose pillows, always thought I was a mouth breather but maybe that was because of OSA!
