Treatment Emergent Central Apnea or Insomnia -- which is it? - mallar - 02-25-2024 I've been on my CPAP for about 2 months. It's been up and down, but I've seen some benefits. My OSA seems to be completely managed now, last week's OA index was .48. The problem is that I still wake a up a lot overnight. I've been to two different providers, and both have given a different interpretation of what's happening. CA index is still somewhat high, last week's was 4.29. Provider #1 suggested my wakeups were because of treatment emergent central apneas and that would either subside over time or be addressed with an overnight titration. Provider #2 suggested that it was simply insomnia (in effect, my body is used to waking up a lot from the previously untreated OSA and is now habitually waking up with the CPAP) and that the CA's are coming shortly after I fall asleep. That provider suggested I should manage the wake ups with behavioral changes known to help with insomnia. Machine pressure: 6-15, full time 2 EPR. No ramp for now, as I find it hard to breathe with just the mask hooked up and no pressure flowing through the machine. I have only had providers and sleep specialists set the machine for now, but I am open to adjusting it myself if needed. I will post OSCAR data from a recent typical night. The devices settings are found on screenshot #2. On the night of the posted data (2/23-24/24), I tried to make a note of when I woke up (on a timestamping app on my phone). I recorded wakeups at 11:57PM, 1:16AM, 2:32AM, and 4:01AM. As this required me to be with it enough to remember to open my phone and tap it for the stamp, it's possible there were more, and that the actual wakeup time was a few minutes earlier. I do notice more events logged around those times in OSCAR. I'm still new to OSCAR and sleep data, and I don't know what to make of this. Which could it be? Are my CA's waking me up, or is it merely insomnia? I'm at a bit of a loss as to where to take my therapy. Is there something I need to alter on my machine? I'm still new to the CPAP and haven't yet altered the machine's settings myself, but I am very open to fine tuning things. |
