First night with CPAP (Luna G3) - Big Ultima Energy - 04-06-2024

Hello - I'm recently diagnosed with OSA and just used a CPAP machine for the first time last night, and am curious if there are any setting tweaks I should make before I use it again tonight. My DME set me up with a Luna G3 APAP. I'll be meeting with my sleep doctor in about 2 weeks.

Here are the specifics:

Sleep study results:
  • REI: 11.6
  • Lowest O2 Saturation: 82%
  • (my primary symptom is excessive daytime sleepiness, along with the mental health issues that go along with that)
Prescription:
  • APAP: 5-15cm H20 with EPR to comfort
CPAP settings:
  • humidifier: auto
  • heated tubing: auto
  • ramp time: auto
  • reslex: 3
Mask:
  • ResMed P10 (nasal pillows, medium)
Results:
  • Usage time: 11 hours (I was awake for the first 2 hours, reading a book)
  • Average Pressure: 7.5
  • AHI: 8.2
  • P95: 9.0
  • CAI: 7.2
  • Average leak: 5.3L/min (I disconnected a couple times to use the bathroom - may have affected this #)
I kept the mask on the whole night, but my overall sleep quality was pretty poor. Lots of tossing and turning - I feel pretty tired today. I was surprised to see my AHI at 8.2, given that I kept the mask on all night.

My nostrils are moderately sore.

When I turned over in the middle of the night some water shot into my nose...maybe I need to set the  humidity lower? The humidity temperature also seemed a lot cooler than it was before I fell asleep.

The air pressure seems to vary a lot through the night. Sometimes it felt fairly strong, and sometimes it didn't feel strong enough.

As a side note: after reading a few posts on this board, I wish I would have known at the time to request a ResMed CPAP. After I meet with my doctor in a couple weeks, I might ask about switching. But one of my co-workers also uses a Luna G3, and she said it works great for her.

Okay, since we can't get the clinical data from it and looking at what you've posted I think you should disable ramp completely (turn off), change heated tube to 1 and humidifier to 3. Raise min pressure to 6.5cm and lower max pressure to 10cm. Let's see if this helps you with pressure fluctuations during the night. Reslex is their form of EPR so if it's set to 3 I think that is the maximum....

Keep your machine lower than your head too, I prefer mine on the bottom shelf of my night stand-and either buy or rig up a hose hanger!

Thanks for the tips! I tweaked the settings and my numbers were much improved. I also switched to the "large" pillow size and put some lanolin ointment around my nostrils, which helped a LOT. I woke up a bunch of times...probably because my brain isn't used to having something strapped to my nose at night. Numbers from last night:

Pressure: 6.0
AHI 1.0
P95: 8.0
CAI: 0.8
