Middle of the night wakeup - SteveC - 04-10-2024

Hi all,

Please see attached screen shots. When I try to go back to sleep after a toilet break, 2am on the attached chart, I have a large number of CA's. 

Can anyone suggest why this may be so?

Steve, you have been on Apnea Board longer than I have, and from the beginning, you have experienced a high AHI, mostly central events. You have tried many permutations of pressure and EPR and your AHI has remained stubbornly high, even considering your original diagnostic AHI was only 14. At this point, you have run out of options with CPAP, and need to move to a more advanced bilevel device to address this. ASV would clearly work to resolve all of these events, and probably give you better sleep. I don't think I can recommend bilevel without a backup rate because you showed such high AHI when you tried CPAP with EPR. I really think your next step is to discuss this with your doctor. With over 12 years of CPAP you have never resolved issues with central apnea or gotten the sleep quality you need to make your life "normal". You need bilevel/ASV titration or a prescription for ASV and all this can be resolved. Alernatively, we can discuss means of self-financing and getting your own ASV with our without assistance from your doctor or insurance.
