Soleil211 - ASV Therapy - Soleil211 - 05-23-2024 Hello all! I finally received my ASV that I’ve had trouble getting approval on (finally!!). Last night I tried using it for the first time, but found I could not. I’m not that familiar with this type of machine. It’s not nearly as straight forward as the cpap. I did check with my provider to make sure the settings were correct because they looked suspiciously like factory settings. They confirmed they are. When I tried to use the ASV last night, the secondary pressure would blow so hard after a few minutes I would start to get chest pains. This is while I was awake. I tried to use it twice and the same thing happened, so I gave up. In conclusion, any assistance I can get with the settings would be very much appreciated. RE: Trouble with my new ASV - SarcasticDave94 - 05-23-2024 OK, a few things to help us with helping you. It will be most helpful for you to get OSCAR from the link at the top of this page. You need an SD card for the ASV as well. If you have one with your old CPAP, this will work but I would suggest saving old data on the computer then erasing the SD card to start over with ASV. Do you have a copy of the free CPAP setup guide for the ASV? Get it if not. Can you tell us your current settings? Do this to access clinical menu: click and hold Home and Dial together for about 5 seconds. There's the clinical menu where you can see and program the settings for yourself. And yes you are permitted and will be able to understand what you're doing with assistance here. You will see the following info which you can tell us: Mode, should be EPAP singular or a range of 2 numbers, PS with 2 numbers, that's all we're concerned about for proper setup. This isn't harder than an AutoSet honestly. RE: Trouble with my new ASV - Soleil211 - 05-23-2024 Ok, so my settings are as follows: Mode:ASVAuto Min EPAP: 4:0 Max EPAP: 15:0 Min PS: 3:0 Max PS: 15:0 I haven’t done Oscar because I can’t get consistent use from the CPAP due to the high number of central apneas. The length of time I’m able to use the CPAP depends on how many central events I’m having. For example, if I’m having events in the 20’s, I can often wear it for several hours or more. But if it’s in the 60’s through 90’s, I can only wear it 1-2 hours tops. But I will wear the CPAP for a week just to gather data if I can’t get the ASV worked out for now. I will work on getting a copy of the CPAP setup guide for the ASV. Your assistance is much appreciated! RE: Trouble with my new ASV - SarcasticDave94 - 05-23-2024 You were thinking default, yep this is default ASV Auto. Try to explain why it's not working and do get OSCAR. I have used the same model ASV and can help change this for better in minutes. You will need to access that clinical menu again to change settings. Trust me it is as easy as clicking, turning to another number, clicking to lock the new settings and then exit the clinical area. Before I mention edits, I forgot to ask about Ramp. Best answer is not using it. If you must, make it about 7 cmH2O pressure and minimal time. Ramp and ASV Central Apnea treatment will oppose each other. Edit to start you off, only one thing now as it is until you can get OSCAR running. Sorry to bring it again but we can't do much running blind. OK enough there Dave, edit EPAP Min from 4 to 7 cmH2O. Nothing else. Try it and report back with OSCAR. Again trust me when I say we can fix this for drastically better in minutes. The edit to EPAP Min 7 may help but there may be more to go. However this will help immediately. PS yes I've been there as well, my CA were doubled by BPAP, terrible feeling to have 280 CA a night with BPAP. RE: Trouble with my new ASV - SarcasticDave94 - 05-23-2024 For now, overnight 1 x. It may be the ASV is a complex machine but it's not harder than CPAP to setup. PS when you're able, go to your Board user info and edit to current ASV machine and mask. Don't worry much about pressures on there just yet. But having ASV listed is helpful to members. I began to get chest pains and heart palpitations and took it off. Honestly, I felt like it was going to send me into cardiac arrest. I’m not certain what’s going on or why I’m having that reaction. Are you familiar with this reaction? I’m thinking it a possible underlying condition that I’m not aware of having. RE: Trouble with my new ASV - SarcasticDave94 - 05-25-2024 It doesn't sound familiar to my usage with ASV, beginning of otherwise. I'm not using one currently due to conflict with COPD/Asthma. If you like, you can edit EPAP Min to 6 to see if that helps out. As always, if you think there's medical attention required, keep your doctor informed, find out any steps they'd suggest. Just to be on the safe side. I have a heart condition called PVCs as well, and it was fine with ASV. RE: Trouble with my new ASV - Rich66 - 05-26-2024 There is a lot of trial and error with an ASV. Default is a terrible setting. Take a big leap, make a big change, and then as you get closer to what works, make smaller changes. If it was me, I'd turn the EPAP Max to 10, and the PS Max to 10 also. You have to see what works for you since everyone is different. And it seems like most people prefer the mode to be ASVauto. If those pressures still cause problems, then turn them down to 8 and try that. Pressures that are too high can cause leaks, headaches, and burps, and who knows what else. |
