New CPAP User - Waking with very high pressures - Westernsleep - 07-05-2024

Hello - Thank you in advance for all of the resources this board and it's members provide.  I'm 5 weeks into using CPAP for "mild" OSA.  I purchased a Resmed Airsense 10 using the Airfit f30 full face mask.  I do feel like the mask straps and cushion are a little too small but I've been sticking with it.  I did order an F30i with a larger frame but couldn't get a larger cushion so it will also be a medium.

On to my problems...I'm finally to the point where I'm falling asleep with the mask on and making it about 1 to 1.5 hours before I wake up with the pressure very high and the mask practically blowing off of my face.  This has happened 4 nights in a row and I believe I would sleep longer without this happening.  I'll take any and all advice related to my waking up or any other settings you recommend I change.  My sleep doctor and specialists were completely non helpful so I'm going this road alone...but I'm committed to sticking with this.  Thank you

Welcome to Apnea Board,

Your max pressure is going to 11.5 versus the max setting of 17, so little issues there.

I would suggest this for your consideration, add 1 to your Min pressure making it 7. Next turn on EPR 3 full time. The 7 pressure is to allow EPR (exhale pressure reduction) the room to reduce to 4, which is the lowest physical pressure this AutoSet goes.

Edit, I saw after the fact you tried Min 8. So take your pick of 7 to 8 min along with EPR 3.

Historically does your max pressure go higher than this sample? I'd say leave the max as is.

FWIW as a comparison my max pressure was 25 and it would hit it sometimes.

16 is about the highest I’ve seen.

Do you have ramp on? If yes, I would turn it off. IME as it ratchets up, it can make it hard to settle down for the night, and also makes the mask fussy. Most people prefer a smooth ride.

Yes, I would decrease max pressure to 13cm, there is no need to have it at 17cm (it's just asking for runaway pressure issues).

I agree with Dave to leave the upper-pressure setting where it is. Westernsleep, you have gone to 16 because you needed 16.

@WesternSleep Please fill out all the info in your avatar, and do it exactly. Use the whole name of your machine that's on the unit. This lets folks see at a glance where you're coming from and makes it easier to get advice.

I agree that a min of 7 or 8 plus EPR of 3 plus no ramp all makes sense.

I think the jury is out on capping the max. I'd want to see the flow limitation graph before offering a recommendation. My suspicion is that FLs are what's driving the pressure up, and higher pressures don't do a heck of a lot for FLs.

Westernsleep, do you have congestion in your nose? Also, the next time you post a chart, could you include the flow limitation graph? You can make room for it in your screen shot by grabbing the gray bars that separate the graphs and pushing them up a little.
